1. Job Role
• The Head of Case Management oversees a team of case workers responsible for resident care coordination. Develops and implements case management programs, including utilization review, intake or discharge planning, and manages the department as per the regulatory practices and policies.
2. Job Description
• Responsible for direction, coordination, management, review, implementation and evaluation of resident case management for the organisation in alignment with the organisation’s strategies and policies and procedures and directions of higher management.
• Manage and supervise a team of case workers and identify opportunities for team training and skills advancement.
• Manage the safe custody, discipline and needs of the residents in the Home.
• Maintain proper records of Admission and Discharge Register, Records of Residents’ Personal Effects and Disciplinary Meeting Records.
• Ensure all case management records, ECRM accounts, CANS and YLS assessments of residents are properly maintained and kept updated.
• Draft and review the Standard Operations Procedure and Best Practices of the Home in accordance to the Standard of Care and policies of the organisation.
• Prepare the Monthly Statistical Report.
• Assist the Head of Home in the Review Committee and Advisory Board meetings, attend court reviews and family conference and answer queries or clarifications from relevant stakeholders.
• Conduct regular assembly sessions with residents on feedback received from relevant stakeholders.
• Liaise with Clinical Consultants, Psychologists, Psychiatrists for consultation sessions where required.
• Manage and oversee the Individual Care Plan for all residents.
• Responsible for the Clinical Intervention components in the Home (include counselling, therapy and care services).
• Work closely with Programme Department and relevant stakeholders to analyse the needs of the residents and to ensure the effective execution of the programme in accordance to the resident’s needs.
• Supervise Trauma Informed Positive Behaviour Interventions and Supports (TIP-BIS) and SDM-PFS implementation in the Home.
• Establish and carry out proper training programs for staff in respect of laws, regulations and policies.
• Carry out any other tasks assigned by the Head of the Home, Deputy Head and higher management.
3. Candidate Requirements
• Bachelor’s Degree in Counselling or Psychology
• 2 years of experience in case management of residents
• Has supervisory management skills and knowledge
• Excellent interpersonal, verbal, and written communication skills
• Ability to work with all age groups
