• Assist the Social Worker in performing Resident Assessment Form (RAF) for potential residents into the HOME
• Update colleagues on residents’ medical and nursing matters
• Advise Head of Home and Deputy Head of Home on admission of potential residents and discharge of residents from medical and nursing viewpoints
• To conduct RAF for residents every six months or upon request from Head of Home
• Accompany residents to clinics and hospitals
• To update colleagues of residents’ upcoming or rescheduled medical appointments
