Job Description:

• Perform intake assessment and review of day care clients and oversee multi-disciplinary meetings for reviews of clients including Day Rehab clients.
• Supervise daily routines such as meal preparation, serving of meals, preparing clients for activities, conducting of activities, reviews of clients and cleaning duties of care staff.
• Work with Centre nurse and care staff to ensure clients specific needs (ADLs) are met and communicated to the team.
• Co-ordinate and plan programmes and services with internal and external stakeholders.
• Direct, guide and participate in the orientation and training of new staff and provide ongoing training and monitoring of care staff in the area of provision of care to day care clients and clients with dementia.
• Take charge of daily operations, and care provisions of clients, supervise and deploy staff to ensure operations of SCC is safe and effective.
• Take charge of incidents in the Centre related to staff, clients and volunteers to ensure that the incident is resolved, reported and follow-up.
• Work with Centralised Team to ensure accurate billing and charges for clients and follow up on staff related matters.
• Work with Cluster Lead to ensure financial sustainability of the Centre.
• Ensure accurate and timely documentation of communications, assessment and processes.
• Carry out other responsibilities and duties as assigned from time to time.

Job Requirement:

• Recognized degree in a healthcare discipline, e.g. nursing or allied health.
• A minimum of 5 years’ experience in clinical area such as acute or community hospital, of which at least 2 years in a leadership position.
• Good knowledge of the healthcare environment and service processes will be an added advantage.
• Experience in community care provision is an added advantage.

ROLE & RESPONSIBILITIES

The Therapeutic Care Worker (Youth Mentor) is responsible for providing trauma-informed care, supervision and management to female adolescents who have experienced abuse or neglect. With an aim to promote wellness and quality of life, the Therapeutic Care Worker (Youth Mentor) ensures that basic personal, physical, emotional, spiritual, mental, educational, and social needs of residents are met.

• Carry out the daily operational duties and supervise residents in their regular activities
• Assist residents with any difficulties related to day-to-day issues at Dayspring Residential Treatment Centre and ensure their welfare and safety
• Promote discipline among the residents and be a mentor/role model to them
• Assess, respond and handle crises or conflicts
• Carry out individual therapeutic interventions based on care plans by treatment team
• Participate in providing physical, emotional, spiritual, mental, educational, and social needs of residents

CANDIDATE REQUIREMENTS

Pre-Requisite:

Youth Mentors are required to work with young beneficiaries with complex trauma. Suitable for workers with cognitive and emotional maturity

• Must be willing to work on 3-shift rotations and over weekend/Public Holidays
• Must be able to work with children and teenage girls
• Licensing Agency, MSF, requires direct care staff to be minimum 21 years of age
