We are looking for dedicated and compassionate Home Care Nurses to join our team and provide essential care to chronic ill and palliative patients at home.

Key Responsibilities:

• Perform home visits to assess and manage chronic ill/palliative patients.
• Conduct nursing assessments and coordinate with doctors for medical evaluations and follow-up visits.
• Monitor patient medication adherence and compliance with doctor-prescribed treatments.
• Perform critical nursing procedures such as wound dressing, catheter insertion, naso-gastric tube insertion, and care of medical tubing.
• Develop and implement nursing care plans and suggest new measures for patient management.
• Educate caregivers/families on essential nursing procedures to ensure continuous care at home.

Requirements:

• Minimum Diploma/Degree in Nursing.
• At least 2 years of specialized training in Onco, Home Care, Palliative Care, Geriatrics, or IPW.
• Ability to work independently with minimal supervision.
• Experience in managing 3-5 cases per day on average

Responsibility:

• Provide education and counseling to expectant mothers on the techniques of breastfeeding.
• Work together with other healthcare providers to ensure coordinated care for mother and baby.

Requirements:

• Minimum Diploma in nursing
• Nurses with lactation consultation experience, preferably with IBCLC cert or advanced diploma in midwifery or with post-natal ward experiences

Working hours:

2 to 3 days Ad-hoc assignment.

Any days from Mon to Sunday

8am to 530pm
