Responsibilities

• Provide skilled nursing care at patients' homes under physician direction.
• Assess patient needs and develop care plans.
• Administer medications and treatments.
• Monitor patient health and vital signs.
• Educate patients and families on disease management.
• Coordinate with healthcare team for comprehensive care.
• Document patient progress accurately.
• Ensure compliance with standards and regulations.
• Maintain patient confidentiality.
• Handle medical emergencies promptly.
• Communicate effectively with patients and families.

Requirement:

• Palliative / Home Care / ICU / ED Trained ( Min 5 years )
• SNB and BCLS Valid

Additional Info :

• 5 days, 8.30 – 5.30pm Office Hours (No on call)
• One day they will be covering 3-4 homes depends on the patient loads

Job Description

• Assess each patient's physical, psychological, and social needs to determine priority of care.
• Working closely with other nurses and doctor in urgent situations accurately assessing the patient condition and prioritising care reviewing medical histories executing treatment plans.
• Provide timely and accurate assessments of a patient's medical history, chief complaint, and medications.
• Ensure patient comfort while providing safe, patient education care.
• Perform resuscitation, CPR when necessary.
• Provide assistance to doctors during physical examinations, diagnostic tests, and patient treatment.
• Administer medication.

• Support SNs in patient’s individualised care plan
• Support with nursing procedures and nursing care (i.e. post-surgery care, medication administration)
• Provide general caregiving and personal care (i.e. companionship, assistance with activities of daily living)
• To ensure accuracy of all documentations related to patient care in the system.

Requirements:

• Min Nitec in Nursing
• Valid SNB and BCLS
• Min 2 years of acute ward experience or homecare exp
