• Provide structured nurse counselling, health screening service, patient education on mental health management, chronic disease and health promotion.
• Understand patients' chronic conditions and improve their health outcome.
• Advise patients on how to administer insulin injections, perform self- monitoring of glucose and home blood pressure readings, proper usage of inhalers, provision of targeted diet plans.
• Conduct eye and foot screening with patient education to diabetic patients.

Requirements:

Job Scope

• Assesses, plans, implements and evaluates infection control practices of nursing staff, doctors and all healthcare workers in organisation
• Conducts teaching for nursing, paramedical and medical staffs and students in infection control practices and guidelines

Requirements

• Registered Nurse with at least 9 years of post-registration clinical experience, of which 2 years must be as Assistant Nurse Clinician
• Diploma in Nursing with relevant post-basic advanced certification and Degree in Nursing from recognized training institution as approved by Singapore Nursing Board, and certification / prior working experience for a year in infection control.
• Technically skilled, competent and able to work independently
• Good interpersonal and communication skills
• Computer literacy skills –MS Word, MS Excel and PowerPoint
