About the Role

Job Summary:

The incumbent is part of TOUCH Special Needs Group (TSNG) who reports to the Centre Manager of TOUCH Ubi Hostel (TUH) / Community Support Service (CSS) and is responsible for the role of Social Worker I that encompasses implementing holistic services and programmes that will benefit and make an impact to the needs of clients under TSNG and CSS. The incumbent is responsible for delivery of quality service, personal efficiency and teamwork at the workplace

Main Responsibilities:

• Planning and executing community outreach programme and/ or support group for clients in CSS. Conduct feedback and outcome-based evaluation for programme.
• Mobilise resources and work with community partners to support the execution of community programme.
• Lead and mentor volunteers in planning and execution of CSS activities.
• Outreach to community partners to advocate for Persons with disability (PwD) and recruitment of potential CSS clients.
• Handle enquiries pertaining to programme and to assess potential clients for intake to CSS or other services under TSNG.
• Casework management for CSS clients who need support such as social-emotional support, long term care planning etc.
• Provide service/ care coordination of PwDs and caregivers from information gathering, intake assessment and case coordination with other stakeholders/agencies
• Outreach to PwDs in the community through various means such as door-to-door or roadshows etc.
• Build and maintain strong relationship with families and caregivers and organize and conduct caregivers’ support activities under TSNG.
• Work with the team in the transition planning for trainees graduating from TSNG’s services into the community.
• Work with staff from various services for special project on ad-hoc basis.
• Maintain and update membership application and renewal under CSS
• Produce programme/ event reports for documentation and submission to funders.
• Produce social report and case notes under case management.
• Maintain proper case note recordings and documentation.
• Draft articles on clients for newsletters or publicity when needed.

Essential Skills and Qualifications:

• Degree holder or Graduate Diploma in Social Work.
• Registered Social Worker under Singapore Association of Social Work.
• Preferably with 1-2 years of social work experience in working with person with special needs.
• Proficiency in Microsoft Office applications.
• Passionate and dedicated to work with the special needs community.

As the Facilities Maintenance and Operation, you will be key person in handling and maintaining the operational readiness of the 3 gallery spaces in Tanjong Pagar Distripark and 2 additional retail spaces in Paragon.

Key Responsibilities:

· Key person in the facilities management of all the gallery and retail spaces to ensure operational readiness of the business

· Set up the gallery display, including hanging the paintings, moving sculptures and pedestals, adjusting lighting, etc

· Maintaining productive relationships with suppliers

Requirements:

· Min. 1-2 years of experience in operations and/or facilities maintenance

· Diploma or graduate in any arts management, facilities management or related courses

· Meticulous with great attention to details

· Customer orientated with good interpersonal and communication skills

· Solid negotiation skills

· Resourceful and able to work independently

· Able to work under tight deadlines

· Highly motivated and able to undertake pressure

· Knowledge on inventory control is an advantage

1. Assessment (Social Investigation)

• Schedule investigation interviews promptly with the Intake Centre and client.
• Interview the client and relevant persons / stakeholders to make a reasonable and fair assessment of the client.
• Prepare the social investigation report for vetting by the supervisor.
• Right site the client to a community setting or welfare home within the DPA (30-day timeframe).

2. Casework

A. Engagement & Assessment

• Develop professional & engaged relationships with clients and other key service stakeholders (e.g. Family Service Centres (FSCs), Social Service Offices (SSOs), Ministry of Social and Family Development (MSF), etc.)
• Conduct mentoring sessions for all assigned clients as per the service model requirements.
• Conduct needs assessment and risk assessment for complex cases and refer to the relevant agencies for intensive case management.
• Co-manage complex cases with the relevant agencies regularly.

B. Case Review & Documentation

• Prepare social reports / case recordings following the requirements of New Hope CS and ensure documentation is in place.
• Ensure the database is timely updated.
• Consistently collect relevant documents from clients to prepare for submission to the funder at the end of the contract period.
• Meet the deadline for submission of reports.

C. Supervision

• Attend individual and group supervision as scheduled regularly.
• Meet the supervision hours required by the funder.

3. Candidate Requirements

Technical Competencies:

• Degree in Social Work or Post-Graduate Diploma in Social Work from an accredited institution
• Accredited as a Registered Social Worker with SASW
• At least 2 years of Social Work / Case Management experience is preferred.

Core Competencies:

• Strong interpersonal and communication skills
• Understand the principles of engaging and working with vulnerable families and individuals
• Case management and casework knowledge and skills
• Good report-writing skills
• Willing to work with the homeless and destitute
