Working Hours

Under normal circumstances the hours of duty will be as follows:

8.00am to 5.15pm (Monday - Friday, 45 minutes lunch break inclusive)

or

8.30am to 5.45pm (Monday Friday, 45 minutes lunch break inclusive)

Summary

• Maternity Cover till 31 May 2025
• Monday - Friday (5 Days), Office Hour
• Day Care Centre (west region)

Job Responsibilities

• Build rapport with residents/ clients and their families
• Maintain communication with clients while addressing their concerns and goals
• Provide casework management for clients and coordination with hospitals/ polyclinics for clients who require financial assistance
• Develop and implement individual care plans for clients and provide therapeutic support as needed
• Financial assessment and fund management and prepare periodic reports as required - identifying and referring cases for financial assistance

Requirements

• Degree in social work or relevant
• Great interpersonal and communication skills
• Experience working with elderly is a plus!
