Officer contributes actively to the programme needs of Special Student Care Centre under the umbrella of Community-Based Support Services (CSS).

CSS is the dynamic and nimble service delivery arm in MINDS that response to emerging needs of PWIDs and Caregivers in the community. Examples of services include Me Too! Job Scope:

• Support clinical team in advising clients on financial scheme applications
• Ensure timely update of renewal of means testing and financial assistance
• Support the clinical team in administrative processes in the delivery of services to clients and caregivers
• Manage onboarding process for new clients, assist in initial assessment appointment scheduling/ rescheduling, subsidies application, and address client inquiries for rehabilitation
• Assist in initial assessment of client/caregiver needs with basic case management under the direction of a case manager/ social worker
• Ensure client and caregiver’s ability to cope by helping to streamline and coordinate the range of community services available
• Assist client/caregiver with appropriate information and referral assistance
• Conduct joint home visits with a case manager/ social worker or the multidisciplinary team where necessary
• Participate in case discussion with the clinical team
• Consolidate, prepare, track relevant monthly reports and statistics

Job Requirement:

• Min Diploma in Healthcare or Social Work any other equivalent certifications

The Care Associate (CA) will provide home-based personal care services which aims to assist clients with their daily activities of living, to enable them to be taken care of and live in the comfort and security of their own home.

Duties include but not limited to companionship, personal hygiene, medical escort, grocery shopping, meal preparation, feeding and light housekeeping.

Responsibilities

• Assist with instrumental activities of daily living (“IADLs”), including light housekeeping and laundry, assistance with medication and simple errands such as grocery shopping.
• Provide companionship and engage in mind-stimulating, recreational and leisure activities with client
• Performing simple maintenance exercises as prescribed by a registered therapist.

Requirements

• GCE N Level
• Attained training in either one of the following courses:
• WSQ Higher Certificate/ Advanced Certificate in Healthcare Support (Nursing Care)
• ITE Approved Training Centre Healthcare (Home Care) programme
• At least 1 year of working experience as care staff in the home care setting
• Able to use local languages and dialects.
• Must be client-oriented, self-directed, independent yet able to work as part of a multi-disciplinary team and have good interpersonal and communication skills.
• Have a valid Basic Cardiac Life Support (BCLS) Certification.
• Able to complete visit reports.
