1. Job Role

• The Head of Case Management oversees a team of case workers responsible for resident care coordination. Develops and implements case management programs, including utilization review, intake or discharge planning, and manages the department as per the regulatory practices and policies.

2. Job Description

• Responsible for the direction, coordination,management, review, implementation and evaluation of resident case management for the organisation in alignment with the organisation’s strategies and policies and procedures and directions of higher management
• Manage and supervise a team of case workers and identify opportunities for team training and skills advancement
• Manage the safe custody, discipline and needs of the residents in the Home.
• Maintain proper records of Admission and Discharge Register, Records of Residents’ Personal Effects and Disciplinary Meeting Records.
• Ensure all case management records, ECRM accounts, CANS and YLS assessments of residents are properly maintained and kept updated
• Draft and review the Standard Operations Procedure and Best Practices of the Home in accordance to the Standard of Care and policies of the organisation
• Prepare the Monthly Statistical Report
• Assist the Head of Home in the Review Committee and Advisory Board meetings, attend court reviews and family conference and answer queries or clarifications from relevant stakeholders
• Conduct regular assembly sessions with residents on feedback received from relevant stakeholders
• Liaise with Clinical Consultants,Psychologists, Psychiatrists for consultation sessions where required
• Manage and oversee the Individual Care Plan for all residents
• Responsible for the Clinical Intervention components in the Home (include counselling, therapy and care services).
• Work closely with Programme Department and relevant stakeholders to analyse the needs of the residents and to ensure the effective execution of the programme in accordance to the resident’s needs.
• Supervise Trauma Informed Positive Behaviour Interventions and Supports (TIP-BIS) and SDM-PFS implementation in the Home.
• Establish and carry out proper training programs for staff in respect of laws, regulations and policies
• Carry out any other tasks assigned by the Head of the Home,Deputy Head and higher management

3. Candidate Requirements

• Bachelor’s Degree in Counselling or Psychology
• 2 years of experience in case management of residents
• Has supervisory management skills and knowledge
• Excellent interpersonal, verbal, and written communication skills
• Ability to work with all age groups

Responsibilities

• Managing all frontline operation in an efficient manner.

The Warehouse staff in a GDP and GMP secondary assembly area shall work closely with the warehouse team to maintain proper storage conditions, handle products safely, and adhere to GDP and GMP requirements to ensure product quality and integrity. The key responsibilities shall include below:

• Receipt of Products: Receive incoming products and verify the accuracy of shipments against customer pre-alert and provided details. Inspect incoming products for damage, contamination, or discrepancies and report any issues to the appropriate personnel. Ensure that all received products are properly labelled, tagged, and stored in designated areas.

• Storage and Handling: Store products in accordance with labelling specifications, ensuring proper temperature control and segregation of materials. Maintain cleanliness and organization in the warehouse to facilitate efficient storage and retrieval of products.

• Pick and pack for Outbound processing including Pick and pack products in accordance to customer instructions.

• Secondary Assembly: Perform secondary assembly operations on pharmaceutical products based on customer requirement.

• Inventory Management: Work with the warehouse team and perform regular inventory counts and reconcile inventory records to ensure accuracy.

• Compliance Monitoring: Monitor warehouse operations and report any non-compliance. Implement corrective and preventive actions as needed to address non-compliance issues and improve warehouse processes. Participate in execution of quality management system such as change control, risk assessment, CAPA handling and other elements of QMS as may be assigned.
