MAJOR DUTIES AND RESPONSIBILITIES

(A) OPERATIONS AND CLINICAL (80%)

1. Be clinically and technically competent in performing angiographic (Cardiac and Vascular) and additional modalities including SPECT scan where necessary.
2. Perform and assisting to cardiologist and vascular interventionist all angiographic examinations on patients with clinically accepted standards described in the SOP; with due regards to radiation safety, patient care and clinical requirement.
3. Participate in the maintenance of a round-the-clock angiography service by rotating for emergency stand by on call.
4. Being on-call for relief staff for emergency case for both cardiac case and vascular case by roster.
5. Ensure good working condition of the angiography machines and other machines within the cneter including ultrasound and SPECT machine by performing regular quality assurance checks and control, notifying vendors and senior staff of equipment problems.
6. Observe all radiation safety protection rules with regard to patients, staff and the public.
7. Responsible for the good housekeeping and inventory of his/her workstation.
8. Ensure maintenance of proper records of cases done consistent with good angiography practice.
9. May perform more advanced angiographic and SPECT imaging with relevant experience and training.
10. Provide guidance for quality control, practical teaching of students and juniors in patient care and clinical areas of angiography.

(B) GENERAL (20%)

1. Assist in patient management such as preparing, explaining, appointments, statistics, and sending the necessary angiographic image in the system.
2. Supervise junior staff working with him/her.
3. Assists in data collection.

JOB REQUIREMENTS

You will also have opportunities to run ambulatory clinics such as Anti-coagulation Clinic, Cardiology Pharmacist Clinic, Renal Pharmacist Clinic, Smoking Cessation Clinic, Memory Clinic, Geriatric and Cardiology Integrated Medical Clinic. You can even provide medication review service at the Nursing Homes or Patients’ Home.

Requirements;

• Candidate must possess at least a Bachelor's Degree in Pharmacy/Pharmacology or equivalentRegistered Pharmacist with the Singapore Pharmacy Board

1.Clinical Competencies and Duties

In Speech Therapy services, the Speech Therapist must have the general clinical competences below (though not
exclusive to this list) and had attained the specific area of competency for their specialty area. Depending on current
rotation, the Speech Therapist must also develop specific competencies in the management of patients in one of
these disciplines:

• Neurology/ Neurosurgery
• Geriatrics
• Intensive Care Units
• Ear, Nose, and Throat
• Paediatrics
• Others

2.Supervisory Role

• Assist with supervision of TA
• Assist with appraisal of TA
• Assist to supervise personnel, other than NYP students, attached to the department as and when required

3.Quality Improvement

• Adhere to patient quality service
• Contribute actively to department’s WOW, Kaizen or other service quality improvements projects and initiatives
• Report to Senior Physiotherapist/ Manager, seeking advice and providing feedback on service delivery and improvement

4. Research Contribution

• Collect research data as required and assigned by supervisor/ manager

5. Others

• Perform other duties assigned by Supervisor/ Manager
• Occasional contribution to team/ departmental projects outside of office hours as determined by reporting officer
• Adhere to the policies and procedures of the Hospital and Department

REQUIREMENTS:

• Recognized Degree in Speech Therapy
• Graduate with 1-3 years of experience with competencies in the relevant clinical disciplines, required by AHPL
• Registered with Allied Health Professional Council (AHPC)
• Member of the Speech-Language & Hearing Association of Singapore (SHAS) or equivalent (highly recommended)

STATEMENT OF PURPOSE

Works with relevant clinical departments to provide assessment and intervention in management for patients care

MAJOR DUTIES AND RESPONSIBILITIES

Specific (100%)

A. Casework

1. Interview/counsel and conduct biopsychosocial and financial assessment of the patient/ resident and recommend intervention plans to the multi-disciplinary team to establish holistic and personalized care plans
2. Participate and present case in multi-disciplinary team/ multi-agency case discussion to formulate common care goals
3. Coordinate with service providers to ensure patients and families receive timely care support after discharge and make post discharge follow-up call to patients/ families when appropriate
4. Provide information and put up application on services/schemes and financial assistance for example, Medifund, Medication Assistance Fund and Remission etc. Make applications for external funding
5. To escalate and handover case management to MSW timely when identify suicide/ family violence/ addiction/ abuse case related
6. Make home visits for care assessment when necessary.
7. Timely documentation of patient's biodata, interviews, telephone calls, payments, and field visits for records
8. Provide and reinforce education, information on patient’s condition if required.

B. Group Work

1. Assist in planning, co-facilitating, monitoring, and evaluating of group work
2. Target and reach out to individuals and/ or groups who may benefit from group work

C. Training and Education

1. Participate in quality improvement projects

D. Programme Development and Implementation

1. Participate in hospital committee and/ or work with community partners/ external agencies to improve the quality of care and services to patients/ residents
2. Assist in the review and implementation of programmes for targeted group of patients/ residents

E. Research

1. Participate and support research data collection/design/analysis
2. Assist to develop abstracts/posters/oral presentations

F. Others

1. Advocate for the protection of patient’s dignity, confidentiality, and rights
2. Actively involved in hospital and department’s initiatives
3. Embrace continuous improvements as our way of life by improving and refining work performance daily. Participate in Kaizen projects.
4. Be a CARE team member in times of civil emergency or mass casualty
5. Any other duties as assigned by Supervisor/ HOD

JOB REQUIREMENTS

(A) Education, Training and Experience

1. Must have attained professional qualifications either in nursing, medicine, or para clinical disciplines
2. Must have prior job experience in either nursing, medicine, or para-clinical disciplines that commensurate with the job
3. Must have at least 5 years working experience in patient related care
