8. To vet and endorse the minutes of all safety-related meetings and other meetings as directed by the Developer.
9. To ensure the implementation and monitoring of appropriate safety measures in accordance with the Workplace Health and Safety Act.

RESPONSIBILITIES

Our Medical Social Workers collaborate with a multi-disciplinary team of stakeholders to regularly review individual residents' medical, nursing, therapy, and social aspects as part of delivering person-centric care. They will be involved in the means-testing review, and provide fair payment proposals, financial application assistance, and any appropriate assistance based on the social & economic status of the family and/or resident.

Admissions

• Provide financial counseling upon admission
• Co-ordinate an assessment on site of the patient to assess nursing/rehab status/behavioral and any social concerns

Case Conference

• Review social aspects inward with regards to activity participation, visitors and befriending, financial consideration status, etc.
• Provide therapy/nursing/medical/social recommendations and support family and resident to complete home discharge or other care options
• Review supporting documents for referral to other care options and seek financial assistance for home equipping as recommended by the therapist where required
• Provide counseling to patients with psychosocial issues
• Facilitate family conferences for discussion on financial considerations, if necessary
• Work with the Nursing team to review care for residents and understand their social situation

The Quality & Training Team in Primary and Community Care Division supports the capability and capacity building of the PCN HQ care teams to strengthen team-based care approach. RESPONSIBILITIES

1. Patient Care Management- To support Clinic Doctors to

• Assess and take comprehensive history of patients under her charge.
• Perform physical examination of patients under his/her charge.
• Develop and implement plan of care.
• Perform follow-up review of patients under his/her charge; coordinate and evaluate care using appropriate care plans.

2. Patient education

• Provide specialised patient education / caregiver training.Implement and coordinate individualised and group learning programmes with patients/caregivers.
• Evaluate effectiveness of patient education; perform health education and discharge teaching to patients/families/caregivers.
• Communicate treatment plan with patients/ families.

3. Infection control

• Keep up to date to COVID 19 measures and practise standard precautions when in contact with patients.
• Assist in monitoring and upholding of infection control practices e.g. standard precautions and hand hygiene.
• Educate patients and relatives on infection control awareness and practice.

4. Staff training & development

• Provide clinical guidance and direction to Junior Enrolled Nurses, Nursing Aides and Care Associates in the delivery of patient care.
• Continuous professional development.
• Maintain a high level of professional self-development through training, supervision, learning and research, linked to continuing registration requirements.

5. Programme Management & Development

• To assist in the Clinic's Specialist Screening programmes.
• To plan and conduct training workshops with Specialist Doctors, Allied Health Professionals such as Psychologists, Social Workers, Physiotherapists, Occupational Therapists to raise awareness of disability work.
• To support the Head of Clinic in clients' case review meetings.

6. Administrative & Operations

• To coordinate staff meetings as and when required.
• To provide timely and accurate monthly dashboard reports as required to the Head of Clinic and relevant MINDS Department.
• To participate in developing, customizing, implementing, and reviewing clinical standards and PDPA and referrals SOPs.

7. Volunteer Management & Public Education

• To supervise and evaluate the volunteers' involvement initiatives, if any.
• To facilitate public relations activities to promote the branding and image of the Clinic.
• To support the Head of Clinic in coordinating regular caregivers' interaction sessions.

• Registered Staff Nurse (SNB License) with minimum of 2 years of clinical experience.
• At least a Diploma in Nursing.

• Technically skilled and competent.
• Good interpersonal and communication skills.
• Computer literacy skills MS Word and PowerPoint.
• Experience working in the social services or health sector preferred.
• Demonstrated ability to work effectively and efficiently with minimal supervision.

This role focuses on managing the needs analysis, innovation and implementation of new delivery models and pilots to better meet the emerging care needs and capability development plans in Dementia Care.

Manage the research and innovation of new delivery models and pilots to better meet emerging needs and new trends.

• Conduct research and horizon scanning to identify emerging trends, service gaps, and project needs.
• Drive the engagement and partnership with stakeholders to identify opportunities for collaboration that is in line with the Community Mental Health Division’s work-plan.
• Manage and engage the clinical experts and clinical team to provide clinical inputs to Ministries/agencies and other Divisions for the purpose of policy review and service planning.

Manage the development of training courseware, additional support and resources on dementia care topics.

• Lead the team in conducting a landscape review of unmet capability needs and identifying specialised training opportunities.
• Design and develop new training courseware (in different modalities) to equip CMH partners with specialised knowledge and skills.
• Engage the subject matters experts and the training workgroup to collaborate on training courseware development.
• Develop the Train-the-Trainer model to step up training delivery.

Develop and implement appropriate caregiver support services and enablers in the community care sector, to better support caregivers of persons living with Dementia.

• Lead team in review of caregiver programmes and assessment tools, and evaluate the feasibility of incorporating successful caregiver support elements into other existing Dementia care services.
• Lead team in the research, design and pilot of appropriate support models for caregivers of persons living with Dementia (e.g. night respite), to enhance accessibility and relevance of services for caregivers.

Job Requirements:

• Bachelor’s Degree in healthcare related discipline, with preference for Dementia care and training qualification.
• At least 8 years’ experience in healthcare or community care sectors with 2 years in managerial capacity.
• Prior experience in Dementia care or geriatric areas is essential.
• Experience in capability development, programme implementation and/or programme management.
• Able to work well with groups from diverse backgrounds.
• Able to multi-task and possess strong analytical, problem solving and project management skills.
• A self-starter with strong learning ability, who is resilient, adaptable and able to manage change and ambiguity.
• Good interpersonal and communication skills.
