Job Purpose:
a) To provide counselling and psychosocial support for patients and families through case management, in-depth assessment of family dynamics, linking up with required social service agencies to facilitate care and discharge planning, for inpatients and outpatients.
b) To contribute to the community via supporting colleagues in needs assessment, program development, implementation and/or evaluation of programs.
Responsibilities:
1.Clinical and Case Management:
i. Engage and build professional relationship with clients and significant others.
ii. Utilize specialized theories in engaging with clients and significant others (MSW I).
iii. Gather relevant information through intake assessment to determine needs and evaluate psychosocial status of clients and families.
iv. Develop, implement and follow up appropriate care plans with clients and families.
v. Manage and overcome reluctance and resistance to change (MSW I).
vi. Conduct financial assessment to determine needs for financial assistance, when required.
vii. Provide illness- related psychoeducation, counsels and support clients/families experiencing psychosocial issues associated with the illness.
viii. Conduct home visits when appropriate and necessary.
ix. Establishes and maintains a working relationship with the multidisciplinary team through regular conferences.
x. Network, refer and liaise with service providers, agencies to match clients to appropriate community services to ensure continuity of care.
xi. Assist in data collection and support action-based research in accordance with clinical practices.
xii. Adhere to protocols in practice guidelines and integrated care pathways.
2. Professional Development:
i. Engage in continual learning through regular case discussions, supervision and trainings (internal and external).
ii. Provide orientation to new staff.
iii. Guide new and fresh staff (MSW I).
iv. Provide sharing sessions on social work for students and professionals (MSW I).
v. Supervision of social work interns (MSW I).
3. Programme Development:
i. Assist and participate in the needs assessment, planning, delivery, monitoring and evaluating of programs when appropriate.
ii. Support psycho-educational groups for patients and/or caregivers.
iii. Develop and lead in support groups for patients within and outside the organization (MSW I).
iv. Assist in facilitating therapeutic group-work when appropriate (MSW I).
v. Deliver public education talks and conduct training above foundational level (MSW I).
vi. Identify service gaps in response to clients’ needs and make recommendations to improve service provision of programmes (MSW I).
4. Administrative:
i. Ensures proper and prompt documentation of case notes, referrals and other related documents for case management and audit purposes.
ii. Collect and submit regular statistics.
iii. Carry out relevant administrative duties as assigned by Supervisor.
iv. Any other duties assigned by Supervisor.
Requirements:
- Education/ Training/ Certification: Bachelor in Social Work / Post Graduate Diploma in Social Work.
- Experience: Fresh graduates are welcome (MSW II).
- Minimally 5 years of clinical experience for MSW I.
- Personal Attributes:Preferably Registered Social Worker with Singapore Association of Social Workers.
- Good and effective team player.
- Proficient in MS Office.
