The Job

The Talent

The Community Resource, Engagement and Support Team (CREST) of REACH Community Services serves as a first mental health touchpoint in the community. We focus on raising public awareness of mental health conditions and dementia, promotes early recognition of at-risk individuals, provides emotional support to individuals and their caregivers and links individuals to relevant health and social care services when necessary. The Social Work Associate provides the team with daily administrative and operational support for the purpose of CREST service development and implementation. You will attend to all enquiries (phone, walk in), organize the logistics &/or resources to ensure the smooth running of CREST services. Key Responsibilities:

Provide administrative and logistical support to the team in running client-related support and/or other CREST activities.

Position Overview:

Work closely with the Active Ageing Centre (AAC) team members & stakeholders in ensuring the effective operation and delivery of the services within the Active Ageing Centre Care (AACC) which includes Active Ageing Programme, Befriending and Community Screening & Social Connector. Key Job Scope and Responsibilities:

1. Program Development
