As part of the Future Care Planning Team, the Life Coaches play a pivotal role in the training and well-being of PwIDS towards independence. Using a person-centred approach, he/she works towards improving their independent living and confidence to access the community with the final goal of enhancing the clients quality of life.

He/she maintains up-to-date documentation of clients' conditions and learning progress. He/She regularly provides inputs for the development and review of future care plans. A caring, respectful, and patient professional, the Life Coach works with various community stakeholders and partners to advocate for the needs of clients to live with support in the community.

Provide Client Training and Life Coaching Skills

• Provide training and support to clients in managing their community living needs.
• Conduct activities that are geared towards achieving goals in future care plan discussions.
• Guide and teach in navigating resources, financial management skills and travelling to their appointments and activities to ensure safe transfer.
• Guide and Provide clients with information on social support services available at other organisations
