i) To work with the PM to comply with environmental, health & safety (EHS) laws laws, advise PM on environmental remediation measures, carry out site inspections and to engage workers and stakeholders on maintaining good environmental health standards

RESPONSIBILITIES

• Assist with the transfer of patients on and off the transport from the beginning of the trip to the end
• Strict compliance to safety protocols of buckling seated clients and wheelchair clients on board transport.
• Provide door-to-door escort from clients' home to Centre and vice versa

• WSQ Certificate in Healthcare Support (Nursing Care) or relevant certification in Healthcare will be advantageous.
• 1 year experience in healthcare setting and preferably in working with elderly.
• Reliable team member, possess positive work attitude, patience and good interpersonal skills.
• Physically fit and enjoy interacting with elderlyWilling to work split shifts

Work Hours: Monday to Friday Spilt Shifts (6.45am to 10.15am and 2.30pm to 5.30pm)

Responsibilities:

· Involve in the care teams assessment of clients in maintenance day care, dementia day care and community rehabilitative programmes;

· Develop and implement care plans for clients and their caregivers with other care team members;

· Perform assessment of the client’s physical mobility and falls risk;

· Provide appropriate physiotherapy interventions based on assessment of clients;

· Conduct functional mobility, training, including the prescription and use of appropriate mobility and/or walking aids;

· Plan, implement and evaluate individual and group physical activity and exercise programmes;

· Conduct and/or supervise group therapy and exercises for the clients;

· Conduct education and training for client’s caregivers; provides support as required;

· Conduct in-service training or/sessions for care staff and volunteers;

· Maintain documentation of clients’ progress and outcomes of care provision;

· Recommend new strategies to the Manager (FHGCC);

Qualification:

· Diploma/degree in Physiotherapy

· Obtained full registration with the Allied Health Professional Council in Singapore

· Good written and spoken English

· Ability to converse in Mandarin and major dialects is an advantage (in order to conduct education and training for Mandarin and dialect speaking client’s caregivers and provides support as required.)

· A mature individual and a team player with good interpersonal and communication skills

· Possess analytical and problem-solving skills

· Passionate in serving the needy and vulnerable

· Proficient in Microsoft Office – MS Word and Excel
