1. JOB PURPOSE
The jobholder is responsible to provide personalised, and cost-effective care to the patients in the nursing home in accordance to established policies and procedures, standards of nursing care and practices.
2. DUTIES AND RESPONSIBILITIES
2.1 Primary Responsibilities and Duties (80%)
2.1.1 Complies with approved standards of nursing practices.
2.1.2 Assists the RN to supervise care staff and healthcare attendants.
2.1.3 Provides basic nursing care to the patients.
2.1.4 Updates on changes in patient’s conditions and needs.
2.1.5 Ensures care delivery and nursing interventions as outlined in the care plan.
2.1.6 Carries out nursing rounds with the RN.
2.1.7 Supervises care staff to ensure comfort and good personal hygiene of patients under his/her care.
2.1.8 Supervises care staff to check, monitor and record patient’s vital signs.
2.1.9 Maintains accurate documentation of care given.
2.1.10 Assists RN in serving medications to patients and comply with safe medication practices.
2.1.11 Assists and guides care staff in the transfer and positioning of non-ambulant patients.
2.1.12 Supervises care staff in diet serving and feeding of patients that require help.
Supervises care staff in ensuring comfort and good personal hygiene of patients under his/her care.
2.1.2 Assists in the admission and orientation of new patients.
2.1.3 Performs rehabilitation activities such as motion exercise and walking with patients.
2.1.4 Organizes and sends patient for appropriate therapy as prescribed, example occupational therapy.
2.1.5 Co-ordinates visits for medical check-ups and transport arrangement.
2.1.6 Accompanies patient to the hospital/polyclinic for follow-up appointments.
2.1.7 Initiates actions in emergency situations; escalate and provides supports to RN promptly.
2.1.8 Works with RN to ensure a safe and clean environment in the home.
2.1 Secondary Responsibilities and duties (20%)
2.2.1 Participates in clinical service projects.
2.2.2 Participates in infection control campaign activities.
2.2.3 Assumes additional responsibilities as assigned by RN, Nurse Manager.
2.2.4 Participates in training and development of care staff.
