The Care Coordinator is responsible for providing support to the clinical team in the

assessment, case work, programmes and supporting the clients and their families

to resolve personal, social, emotional, and financial issues.

He/ She will provide service coordination management, administrative support,

and operations improvement. Care Coordinator will need to work collaboratively

with colleagues from other departments to ensure a seamless client experience for

our service users.

Job Responsibilities

• Support clinical team in advising clients on financial scheme applications
• Ensure timely update of renewal of means testing and financial assistance
• Support the clinical team in administrative processes in the delivery of services to clients and caregivers
• Manage onboarding process for new clients, assist in initial assessment appointment scheduling/ rescheduling, subsidies application, and address client inquiries for rehabilitation
• Assist in initial assessment of client/caregiver needs with basic case management under the direction of a case manager/ social worker
• Ensure client and caregiver’s ability to cope by helping to streamline and coordinate the range of community services available
• Assist client/caregiver with appropriate information and referral assistance
• Conduct joint home visits with a case manager/ social worker or the multidisciplinary team where necessary
• Participate in case discussion with the clinical team
• Consolidate, prepare, track relevant monthly reports and statistics

Job Requirements

• Minimum Diploma in any discipline, preferably in healthcare or related discipline
• Team player with good interpersonal and communication skills
• Patient, gentle disposition with empathy and strong listening skills
• Able to converse in English and at least one other local language is preferred
• Must be able to commute between our 2 centres at Redhill and Boon Lay

Job Description

1. To assist Missions Registrar to oversee the administration matters pertaining to the Asia-Pacific Institute of International Studies (APIIS). This will include:

a. To manage all groups of volunteers for the continuous running batches.

b. To assist new applicants to settle down every year with all info needed.

c. To manage Zoom class.

d. To plan and organise Graduation Events.

e. To maintain a good relationship with students.

f. To assist APIIS faculty to run the course smoothly.

g. To oversee the multimedia ministry to promote and send out the correct postings.

h. To cascade APIIS vision and values down to student body

i. To assist visiting students for meetup in Singapore with faculty

j. Managing and distributing study materials.

2. Coordinate the needs and training of missionaries passing through Singapore.

3. Assist Missions Director to ensure that users of our premises at DM@360 do so in such a manner that maintains GLCC standards and mission goals.

Qualifications

Minimum qualification - Degree

Proficiency in MS Word, Excel and Powerpoint

Experience in student enrolment is a must

Experience in conducting Zoom classes is a must

Experience in sorting out survey statistics reports is a must

Experience in conducting graduation events is a must

Must have an intimate knowledge of the needs of Bible schools, Christian churches, Christian NGO’s, pastors & church leaders in Philippines

Excellent organisational skills

Strong interpersonal and communications skills

Ability to multi-task

1.The Candidate shall possess at least a Degree in Information Technology (IT), Computer Science or equivalent qualification.

2.The Candidate shall have at least two (2) years of working experience in the IT or Infocomm

industry, with exposure to end-to-end software development lifecycle (SDLC). Experience

with implementation in Singapore is preferred.

3. The Candidate main role is to assist project manager and to complement his/her responsibilities.

4 The Candidate is to be proficient in information gathering and reporting that include:

4.1 Compile and organise project-related information

4.2 Schedule and organise meetings with stakeholders and vendor

4.3 Prepare agendas and take detailed minutes

4.4 Follow up on action items from meetings

4.5 Prepare regular status reports for the project manager

5. The Candidate is to have competency in Project Management tracking tools such as

Confluence, Jira.
