7. Writing medical reports and signing death certificates

Overview of Position

Vascular technologists are a highly specialized group of allied health professionals who perform non-invasive studies using ultrasound and blood pressure measurements to assess for vascular diseases or disorders. They are important members of the vascular and surgical team; they assist physicians in the planning and management of a patient’s treatment.

Local vascular technologists are trained on-the-job. They typically work with a high level of independence as they are required to compile information gathered from a series of vascular related examinations to give a preliminary impression for the physicians. In order to do this, vascular technologists are required to have adequate training in vascular scanning techniques, physiological testing as well as have an in-depth knowledge about vascular disease.

A minimum of two years training is required for a vascular technologist trainee to be competent in all aspects of vascular testing. A professional certification awarded by American Registry for Diagnostic Medical Sonography (ARDMS) as a Registered Vascular Technologist (RVT) is required at the end of training.

Vascular technologists work in a fast-paced and physically demanding environment. Vascular studies typically take between 30 minutes to as long as 2-3 hours depending on the number of scans ordered.

Responsibilities

• Finger/ankle/toe-brachial pressure index studies for evaluation of peripheral arterial diseases. In relatively fit patients, exercise ankle-brachial index can be performed to determine the severity of claudication in the lower limb (pain in lower limb muscles as a result of impaired blood flow) when they are on the move.
• Arterial studies for lower and upper extremities for evaluation of peripheral arterial disease. For patients who present with intermittent claudication, ulcers/gangrene in the extremities etc.
• Carotid (neck) arteries for blockages.
• Screening and surveillance of abdominal aorta, iliac and visceral (renal, mesenteric and celiac etc) arteries for disorders like aneurysm (abnormally large arteries), dissection (tear in the wall of the artery), blockages in the arteries, follow-up for post stent insertion or bypass surgery.
• Pre-operative scan for arterio-venous fistula creation (for dialysis) to determine the patency and lumen diameters of blood vessels in the upper or lower limbs.
• Surveillance study of arterio-venous fistula or graft when difficulties are encountered during dialysis eg. Increase arterial or venous pressures, difficulties in needling etc.
• Venous studies of the upper and lower limbs for deep vein thrombosis (blood clot).
• Varicose veins/venous reflux studies for chronic venous insufficiency, varicose veins, spider veins etc.
• Pre-cardiac or vascular surgery screening/assessment to determine lumen diameters of conduits for harvest. They include great saphenous veins, radial arteries and internal mammary arteries.
• Specialised vascular studies for compression syndrome like thoracic outlet syndrome for the upper limb, May-Thurner syndrome (compression of left iliac vein), nutcracker syndrome (compression of left renal vein), median arcuate ligament syndrome (compression of celiac artery) etc.
• Transcutaneous oxygen pressure (TCPO2) studies to detect viable tissue in the extremities to determine healing of wound or determine optimum amputation level.
• Intra-operative scanning of vessels for difficult cases.

Requirements

• Degree in Biomedical Sciences/Medical Technology/Nursing/Biotechnology /Life Sciences.
• Candidates with Degree in other majors are also welcome to apply. Candidates should have at least a Pass in A Levels Biology and O Levels Physics.
• Candidates with relevant work experience in ultrasound/vascular ultrasound would be an added advantage.

POSITION SUMMARY:

As part of the Regional Medical Affairs Team, the Regional Medical Affairs Manager will act as the regional therapeutic expert for our Consumer Health, Pain and CNS portfolios (subject to be updated as for business and medical needs). This role will also be responsible for clinical operations, real world evidence such as CRO management, clinical trial documents and clinical operation support to affiliates. KEY RESPONSIBILITIES:

Medical Affairs

• Provide medical and scientific support.

• Manage medical information inquiries for the assigned therapeutic area.
• Conduct regional scientific training on the disease and products.
• Lead life cycle management and data generation activities for the regional/corporate brands.
• Support the building of strong regional advocates and expert panels and speakers for the brands.
• Maintain knowledge of relevant regulations and guidelines and disseminate same to clinical project teams.
• Research and prepare training on therapeutic areas and indications of relevance and maintain an awareness of trends and changes of importance in the management of those indications.
• Initiates and maintains interactions with key opinion leaders and investigators in therapeutic areas of interest. Create and implement KOL mapping and engagement plan.
• Attend and act as an advisor to relevant clinical development projects or clinical meetings.
• Create or support the development of medical/scientific materials and slide decks as needed and ensure the content is reviewed and approved.
• Act as backup for pharmacovigilance case assessment as needed. Work together with the Pharmacovigilance team to perform adverse event causality assessment for local, regional and global products as well as partner products

Clinical Operations:

• Leads all stages of the clinical study development process including: feasibility assessments; protocol design; CRF design; ISF investigational site selection; generation of clinical study reports and the development of study-related documents, informed consent documents, study manuals and plans, trial master files (TMF), case report form design, etc. Identify data gaps and the need for phase 4 RWE studies for data generation.
• Provide clinical operation support to the local team as needed.
• Act as regional medical lead for CRO management, conduct due diligence for onboarding of the CRO and participate in CRO qualification and audit process.
• Work closely with CRO and keep track of project timeline, budget and milestones.
• Takes responsibility for and initiates the review of all clinical study documents and reports such as project management plans, data management plans, interim reports, etc.
• Work closely with the regional medical director/associate director to study objectives by working with team members to set project priorities and milestones and resolve project conflicts.
• Proactively identifies and resolves issues, and participates in process improvement initiatives as required.
• Reviews and approves invoices from study vendors, investigators, consultants, etc. and planning of the clinical study budgets.
• Oversee medical aspects of the risk management and risk mitigation strategies.
• Act as data manager to ensure the correctness of the clinical study data and verify the statistical analysis.
• Present clinical study data at regional and global conferences as a poster or oral presentation.
• Work with regional TA lead, regional marketing team, local medical team, and local marketing team on the dissemination of the clinical study data; participate/organize investigator meetings, steering committee meetings, and interdepartmental meetings.
• Ensures GCP, regulatory compliance, internal SOPs and working instructions are maintained throughout the clinical study lifecycle.

KEY REQUIREMENTS:

• At least 5 years of medical affairs and clinical research/development experience within a pharmaceutical or Biotechnology Company.
• A certified Doctor (MBBS, MD) or a post-graduate scientific degree in medicine, biology, PhD, etc. will be an advantage.
• Solid understanding of medical affairs in the pharmaceutical industry with the ability to connect medical affairs and commercial priorities to work with cross-functional stakeholders as well as experience in people management and project-based activity and successful delivery of initiatives are required.
• Excellent written and verbal communication skills and good presentation skills.
• Travel to markets within APAC would be required.

Candidates are expected to:-

• Assume the role of the Head of the deck department and Master’s deputy;
• Ensure cargo operations and related safety measures are implemented;
• Liaise with shore authorities and ensure awareness of Safety Management System;
• Ensure deck crew follow safe working practices as defined in company procedures;
• Identify potential ship hazards associated with deck operations and advise the Master accordingly;
• Instruct Cadets/Junior Officers/Ratings on relevant QHSSE and shipboard procedures;
• Perform stability and related calculations for cargo and ballasting;
• Notify the Master immediately of any defects onboard ship which may affect safety and structural integrity of ship which may put the marine environment at risk;
• Assign Deck Officers/Ratings specific roles and responsibilities with respect to the deck department as defined in the Safety Management System; and
• Investigate non-conformances and apply corrective action(s) in consultation with the Master.

The ideal candidate must:-

• Possess a Certificate of Competency (Deck) Class 2; and
• Possess at least 3 years of experience as a Chief Mate onboard a container vessel.
