Job Description:

You conduct physical assessments, provide both individual and group-based interventions, and educate participants on falls prevention, frailty, and healthy aging. Additionally, the role involves active participation in multidisciplinary teamwork and discussion

1. Conduct targeted and individualized physical assessments for participants upon recruitment and during periodic reviews (training will be provided).

2. Develop and implement exercise programs for participant

3. Train and supervise volunteers and staff at Senior Activity Centres to conduct exercise programs and periodic reviews.

4. Provide education talks on falls prevention, frailty, and healthy aging.

5. Ensure proper documentation of records for referral to community partners.

6. Participate in multidisciplinary discussions with team and partners

8. Foster close and warm relationships with participants, their families, and volunteers, demonstrating care and respect.
