RESPONSIBILITIES

1. Plan, implement and monitor the care process, referral and follow-up workflow.
2. Provide supervision to the team to provide care and support, in alignment with the overallprogramme objectives, adhering to the relevant ethical and legal guidelines.
3. Provide care coordination and service linkage to clients with the appropriate resources and support, including caregiver support.
4. Responsible for establishing the network with other stakeholders like community partnersand/or related agencies to build collaborative partnerships and better facilitate care andsupport for clients and caregivers.
5. Establish and review work and communications workflow including identification, referraland escalation work processes as well as the development/update of the servicemanual.
   1. In charge of the analysis and review of the collated data, and monitoring of the team'sprogress and performance.
   2. iResponsible for ensuring that the claims and data submission to AIC is timely andaccurate.
   3. Ensure programme data and documents are kept and maintained, in accordance to theprogramme requirement, as well as the prevailing personal data protection andsecurity guidelines.
   4. Oversee the training and professional development of the team with relevant competencies toprovide the appropriate care and support for clients and caregivers, aligning to the intended care model.
6. Other ad-hoc assignments at the discretion of the Department Head

• Bachelor's Degree preferably in Social Work/Psychology/Sociology/Gerontology/Counselling
• Minimum 3 years' experience in community healthcare settings in Singapore
• Good knowledge of the geriatrics and community services
• Proficiency in Google Suite applications
• Good communication skills (oral and written)
• Passion in community services and service oriented
• Good judgement and decision making skills
• Good in problem solving
• Good record keeping abilities and attention for details
• Possess 1-2 years of experience in administration
• Meticulous and independent

• Ability to speak English, Mandarin and Malay to communicate with clients that are only proficient in one language would be advantageous

Job Responsibilities

• Tracking project timelines, tasks, and milestones through the company's designated software system. Ensure all aspects of the project are updated, organized, and aligned with project goals, allowing stakeholders to easily access current progress reports.
• Act as the primary liaison between clients, vendors, and internal teams to clearly understand and establish the project scope, objectives, timelines, and deliverables.
• Communicate with team members to discuss project status, potential risks, and issues. Proactively address problems by finding practical and efficient solutions, ensuring the project continues to progress without delays or disruptions.
• Source for materials, services, or resources required for the project. Analyze the quotations provided by different suppliers to ensure cost-effectiveness while maintaining quality standards, helping the company to stay within budget.
• Gather data on the status of the project each week and month, summarizing key developments, completed tasks, upcoming milestones, and any potential risks. Submit these reports to relevant stakeholders to keep everyone informed and ensure transparency throughout the project's lifecycle.
• Assign specific roles, duties, and tasks to team members based on their expertise and availability. Ensure that responsibilities are clear and manageable, while also maintaining flexibility to adjust workloads if necessary.
• Manage the work of project team members, providing regular feedback on their performance. Recognize strengths, identify areas for improvement, and provide guidance and support to help the team improve and meet project goals.
• Ensure the project adheres to industry-recognized best practices and quality standards throughout its execution.
• Ensure all deliverables meet or exceed the agreed-upon quality expectations. This involves quality checks, client sign-offs, and ensuring that all project criteria are fulfilled before submission.

Requirements

• Minimum of a diploma in Civil Engineering or equivalent qualification.
• Possession of a Class 3 Driving License.
• Able to work a 5.5-day work week.
• Proficient in English, both spoken and written.
• Technical knowledge in the milling and patching works.

We are looking for a Project Engineer to manage on-site installations and oversee projects from initiation to completion, ensuring seamless execution and adherence to deadlines. You will be responsible for designing, testing, optimizing, and implementing smart condition monitoring systems to enhance business processes, extend machinery lifespans, and provide solutions for predictive maintenance. Your work will involve both hardware and software aspects of these systems, ensuring seamless integration within an industrial environment.

Job Responsibilities:

• Engage in technical discussions with customers and stakeholders to understand their needs and ensure solutions meet requirements
• Participate in designing, testing, and optimizing automated systems to improve business processes
• Participate in conducting Factory Acceptance Tests (FAT), Site Acceptance Tests (SAT), and technical/software demonstrations at customer sites
• Execute on-site installations from start to finish, ensuring smooth operations and meeting deadlines
• Perform on-site and/or remotely support, ie. hands-on installation, debugging, fixing, testing & commissioning, troubleshooting & root cause analysis of application related issues
• Supporting system testing, configuring PLC network systems, integration of electronic hardware and software, and troubleshooting to ensure proper functionality
• Carry out Preventive/Predictive Maintenance of plant instrumentation, vibration monitoring system, control and electrical system to ensure high reliability and availability (on/off site)
• Providing customers with step-by-step guidance, software training, and technical support & solutions for products (online & on-site)
• Control and maintain project documentation, ensuring accuracy, organization, and compliance with company standards
• Ensure proper documentation of designs, tests, optimization processes, and implementations for all systems
• Coordinate with cross-functional teams to ensure all project deliverables are documented and tracked
• Review OEM technical documents and analyze specifications for compliance
• Prepare the BOM list and technical drawings, including as-built, schematic, and single-line diagrams
• Other ad-hoc duties as assigned to support overall team objectives

Qualifications:

• Diploma/Degree in Engineering, Computer Science, or a related field
• Experience in designing, testing, optimizing, and implementing industrial monitoring systems
• Strong knowledge of system integration, with expertise in both hardware and software components
• Proficiency in AutoCAD, HMI development, SCADA programming, and PLC configuration
• Familiarity with project management principles and hands-on experience with project management tools
• Excellent problem-solving skills and the ability to troubleshoot complex technical issues
• Strong communication skills and the ability to collaborate effectively with cross-functional teams
• Ability to work in a fast-paced, dynamic environment and manage multiple projects
• Ability to start work within a short notice period is preferred
