Key Responsibilities:

1. Conduct home visits and perform night call duties when scheduled.

1. Prepare and write care plans for new patients.
2. Conduct monthly checks and replenishment of medication/medical supply and record controlled drugs.
3. Act as a case manager coordinating and implementing a plan of care for the client with advanced disease referred to the organisation.
4. Work in collaboration with members of a multi-disciplinary team in a setting which provides 24 hours medical care, pain & symptom management and family support.

Key Requirements:

The Choksi research group is seeking a Research Fellow funded by a grant through the C4T Phase 2 No Cost Extension hosted at the Energy Research Institute at NTU (ERIAN). The Research Fellow will help advance foundational knowledge in understanding the stability and synthesizability of porous materials using computational methods. The research fellow will employ electronic structure calculations, first principles thermodynamics, and machine learning methods to develop a paradigm for the predictive synthesizability of porous materials..
