Job Description:
- Administer brief psychogeriatric assessments and brief child assessment and collect basic socio- demographic information for all patients.
- Contact patients prior to their first visit to confirm their attendances, remind them to attend and follow up appointment on time.
- Triage cases, conduct home and school visit when needed.
- Provide psycho-education for patients and their family.
- Assess caregivers' needs with appropriate assessment tools and arrange appropriate referrals for support
- Collaborate with Allied Health team, including Medical Social Worker, Psychologists
Requirement:
- Degree in Nursing, Psychology or relevant discipline.
- Minimum 2-3 years experience
