- Personalized Patient Case Management:Provide health coaching to patients, offering disease and treatment-related information and education.
Follow up with patients on treatment regimens and ensure timely submission of relevant program documents.
Document all interactions accurately in the PatientCare Management System.
- Effective Communication:Make scheduled outbound calls to enrolled patients.
Answer inbound calls and address frequently asked questions (FAQs) related to patient care.
Maintain confidentiality and professionalism during patient interactions.
- Adherence to Standards:Ensure program delivery within appropriate service levels and predefined protocols.
Adhere to case handling time targets and maintain patient data confidentiality.
Follow standard operating procedures (SOPs) consistently.
- Medical Guidelines and Reporting:Stay updated with best practice medical guidelines.
Collaborate with the Operations Manager and medical advice team.
Identify and report adverse events observed during patient interactions.
- Quality Assurance:Create and maintain department SOPs.
Conduct quality spot checks on team members.
Identify training needs based on quality assessments.
Participate as an auditee in internal and external audits.
- Additional Duties:Assist with periodic program reports and audit preparations.
Execute tasks assigned by the Operations Manager or Program Manager.
Requirement
- Min Diploma in a relevant field such as healthscience, nursing
- Excellent communication skills and empathy.
- Detail-oriented and organized.
- Ability to work independently and as part of a team.
- Proficiency in using patient management systems.
