- Identify patients suitable for nurse counselling.
- Evaluate patients' social history, current lifestyle, and dietary habits.
- Work closely with GPs to provide individualized care plans.
- Perform eye and foot screenings with educational components.
- Assess patients' mental well-being and refer them to GPs when necessary.
- Assist the administrative team in tracking and monitoring clinical and outcome indicators for the clinics.
- Coordinate with other staff to schedule patients for ancillary services.
- Support the Primary Care Network (PCN) in quality improvement projects aimed at achieving better clinical and process outcomes.
- Other duties assigned by superiors.
Job requirements:
- Advanced Diploma in Nursing and above.
- Min. 4-5 years of relevant experience.
- Registered Nurse.
