Responsibilities:
• Assist in carrying out administrative work in the delivery of advisory services to patients and their families.
• This includes processing of information and statistics on assistance through an electronic system. Executive care advisory may be required to gather information from patients and their families and assist them with the financial assessments procedures.
• Conduct assessment / home visits when necessary.
• Liaise with relevant agencies / authority / patients family.
• Assure that all referrals are attended to by relevant personnel.
• Liaise with nurses on patient-related matters.
• Assure that all referrals are attended to by self or relevant personnel.
• Supervise the operational work carried out by customer care coordinator when handling subsidy cases.
• Act as a channel of communication between management and the customer care coordinator.
• Handle difficult customer and communicate escalated issues to the Manager.
• Keep update records of patients electronic or written data)
• Up-to-date the relevant information of government’s policy with regards to the elderly care Services and funding.
• Maintain confidentiality of customer database.
Requirements:
- Min Diploma & above in Healthcare or any field of study
- Min 2 years of related healthcare customer / patient services experience
- Able to work on alternate Saturdays (off-in-lieu will be provided)
