Staff Nurse (Elderly and Palliative Care) - Tampines

Metta Hospice Care (慈光安宁居家护理中心) is one of the essential services provided by Metta Welfare Association. As a member of the Singapore Hospice Council and the Asia Pacific Hospice Network, Metta Hospice Care offers free home hospice care to terminally ill patients of all ages and races. Established in June 2000, we have assisted over 5,000 patients in living with dignity and quality during the last phase of their lives at home. We care for patients referred by medical institutions and general practitioners, often those diagnosed with life-limiting illnesses like cancer and end-stage renal failure.

We provide comprehensive care, including medical consultations, nursing assessments, and psychosocial support for both patients and their families. Our Social Worker offers additional emotional and social support when needed.

Job Title: Staff Nurse (Elderly and Palliative Care)

Main Responsibilities:

  • Perform home visits for palliative care and home care patients with chronic illnesses.
  • Conduct comprehensive nursing assessments for patients.
  • Develop and implement personalized nursing care plans.
  • Adjust medications to optimize symptom control under the guidance of doctors.
  • Collaborate closely with the Medical Director and outsourced doctors.
  • Educate caregivers and families on basic nursing procedures to ensure patient comfort.
  • Refer patients to inpatient hospice or other professional bodies as needed for symptom control or respite care.
  • Identify and refer patients and families in need of financial assistance to social workers or counselors.
  • Direct patients and families in need of specialized counseling to relevant professionals.
  • Prepare families for approaching death and provide necessary emotional and bereavement support.

Other Responsibilities:

  • Participate in fundraising activities when needed.

Requirements:

  • Minimum of 3 years of related experience.
  • Diploma or degree in Nursing or equivalent, with a Nursing Certificate in Palliative Care.
  • Registered with the Singapore Nursing Board.
