Roles and Responsibilities

1. Referral

  • Timely management of referrals and clarify details with referring agencies.
  • Conduct eligibility assessments for referrals with referring agencies.
  • Ensure timely submission of intake assessments and required documentation onto the necessary systems.

2. Case Management

  • Build professional relationships with fellow professionals, ministries, stakeholders, and service users.
  • Assess needs and risks for shelter cases.
  • Develop risk management and intervention plans for cases.
  • Collaborate on complex cases with relevant agencies through regular case reviews or conferences.
  • Record all necessary documentation and update data records promptly.
  • Organize required documents for reporting.
  • Create and implement shelter programs for tenants.
  • Conduct regular sessions and case plan reviews with tenants.
  • Participate in case conferences and conversations with a multidisciplinary team for complex cases.

3. Discharge Planning

  • Plan for the discharge of shelter tenants.
  • Coordinate and apply for supportive schemes for tenants.
  • Assist in integrating tenants into their new environment.
  • Connect cases with relevant agencies for ongoing social support.
  • Collaborate with stakeholders and community partners for case handovers.

Desired Skills and Competencies

  • Bachelor's degree in social work.
  • Accredited as a Registered Social Worker with SASW.
  • Preferably have at least 3 years of direct social work experience.
  • Proficiency in IT systems and the Microsoft Office Suite (e.g., Microsoft Word, Excel, PowerPoint, etc.).
  • Strong interpersonal and communication skills.
  • Understanding of engaging and working with vulnerable families and individuals.
  • Knowledge and skills in case management and casework.
  • Proficiency in risk assessment and risk management.
  • Strong report writing and conflict management skills.
  • Willingness to work with the homeless.
  • Ability to provide supervision for staff (only for senior position).
