• Develop individualise care plans for residents.
• Execute the care plan and monitor progress.
• Coordinate care for residents with relevant stakeholder.
Job Requirements:
- Min Bachelors Degree in Social Work from a recognized institution
- Min 1-3 years’ work experience in social work, preferably in a social service or healthcare setting would be good
- Candidates with less experience will be considered, entry level
