Responsibilities
- Make home visit to patient home
- Provide chronic disease, activities of daily living and/or palliative advice and counselling, as well as nursing procedures such as nasogastric tube insertion, catheter management and wound care.
- Write case report and update sytem on patient condition
- Liaise with relevant personnels and tap into resources to provide for patients' needs
- Provide emotional support for patients and family members
Job Requirements
- Min. Diploma/ Degree in Nursing
- Registered licence with Singapore Nursing board (SNB)
- Min. 3-5 years of relevant perm acute wards / hospice experience in gerontology and / or palliative care preferred
- Ability to work both independently as well as in a multidisciplinary team, families and caregivers
- Service oriented with good communication skills and a love for clinical care delivery.
