Responsibilities
- Provide post discharge follow-up via telecommunication and telehealth monitoring under the supervision of a Nurse Clinician.
- Provide post discharge phone calls to patients.
- Provide health screening to the residents in the community and health post.
- Work with nurses to develop and manage care for the patients or residents in the community.
Requirements
- Diploma or Degree (preferably in health related discipline)
- Prior relevant work experience as Allied Health Personnel will be an advantage.
- Fresh graduates are welcome to apply
- Good knowledge of various community services and support in the community setting will be an advantage
