Roles & Responsibilities
The Social Work Assistant / Associate will provide psycho-social assessment, social work intervention and support to clients and their families to resolve personal, social, emotional, and financial issues.
Duties and Responsibilities
Client Visitation
· Visit clients at their homes to understand their state of emotional and physical health
· Befriend clients and their families
· Accompany clients to medical appointments where required
· Advice clients on other available resources where appropriate
· Guide volunteers and acquaint them with clients during visitation
Client Engagement
· Follow up with non-active clients via phone calls or newsletters
· Encourage clients to participate in 365CPS’ programmes and services
Administrative Support
· Assist clients in applying for financial support from 365CPS
· Prepare reports on visitation updates
· Maintain database of clients
Job Requirements
· Preferably 1 to 2 years of relevant experience in social service sector
. Diploma in Social Work, WSQ Diploma or Certificate in Social Service
· A team player with good interpersonal and communication skills
· Patience with empathy and strong listening skills
· Proficient in Microsoft Office
