Location : Pulau Tekong
Position : Registered Social Worker Executive
Contract Duration (state estimated start and end date) : September 2024 to August 2025
Working hours : 9am to 6pm, Monday to Friday
You will be working primarily on Pulau Tekong at the Basic Military Training Centre, where your primary clients are full-time national servicemen (NSF) recruits.
Responsibilities:
1. Client Assessment and Triage:
a. Conduct initial assessments of clients’ needs and challenges.
b. Triage and prioritize cases based on urgency and severity.
c. Collaborate with other professionals within and outside of the SAF to ensure holistic support and appropriate case management.
2. Referral Management:
a. Identify appropriate resources and services for clients.
b. Refer clients to specialised programmes, agencies, or professionals.
c. Monitor progress and adjust referrals as needed.
3. Case Management:
a. Identify cases that require earlier case management, or case management that would be better sited within the SAF.
b. Formulate and manage these cases accordingly.
c. Connect, refer, and coordinate the case management of these clients with various parties where appropriate, and at appropriate time points.
d. Provide written reports of higher-risk clients detailing assessment, solutioning, progress, follow-up plans and various discussions.
e. Highlight high risk clients to commanders and medical team.
f. Perform home visit or social visits where necessary.
Requirements:
• A minimum of a Bachelor’s degree in Social Work is required. The salary ranges from $3,500 to $5,500 per month, depending on the years of relevant work experience

• Experience in case management
• Good communication skills, good interpersonal skills
