RESPONSIBILITIES
- Support the social work team in financial assessment and processing of financial scheme applications
- Support the team in administrative processes in the delivery of services to patients and caregivers
- Plays a leading role in compiling of reports and submissions as required by the team
- Assist in initial assessment of patient/caregiver needs with basic case management under supervision
- Assist patient/caregiver with appropriate information and referral assistance
- Conduct home visits independently or joint visits with Medical Social Worker (MSW) or multidisciplinary team where necessary
- Support events or projects in relation to patient and caregiver support as planned
- Any other related duties as directed by the Reporting Officer
JOB REQUIREMENTS
- Possesses a Diploma preferably in Social Work or related field
- Has an interest to pursue/ in the midst of pursuing a Degree or Graduate Diploma in Social Work
- Proficient in spoken and written language, and also effectively bilingual in a mother tongue language (knowledge of dialects or of Malay or Tamil would be an advantage)
- Proficient in Microsoft Word, Excel and Powerpoint
- Good administrative skills would be preferred
- Good knowledge of community services would be preferred
- Positive learner, with a caring and compassionate attitude for the patients and next-of-kin
- Team player with good interpersonal communication skills
- Experience of at least 2 years in the healthcare or social service sector will be an added advantage

- Comfortable with using tech-enable devices
