Teaming with nursing colleague, therapists, Psychologist and therapeutic programme executives in knowledge management to enhance the evidence-based practices, on evidence-based person-centred care and development of the integrated dementia care model to re-frame dementia
Responsibilities:
- Managing assessments and evaluation of integrated approaches for PMCI and PWDs of varying potential and abilities to live in wellness
- Provide leadership in strength-based Living Well Planning
- Provide leadership in psycho-social care and unique models of care for different profile of PWD
- Lead multi-disciplinary team
- Support in review and enhancements of care planning, activity and harnessing environment to empower PWD and PMCI in the community to live in wellness
- Synergise with knowledge partners and co-creators for on-going transformation in integrated person-centred care
- Co-create knowledge from everyday best through documentation of good practices, build tested ground-up models for publication and sharing at conferences.
Requirements:
- A Bachelor Degree in Psychology with Honours
- Excellence in conceptualization, writing and planning strategically leading to quality implementation, sense-making of data and documentation
- Experience with dementia population will be an added advantage
- Strong believer in Person Centred Care
- Dynamic, resourceful, driven, visionary, able to work independently and a great team player
- Excellent interpersonal skills
