We are looking for a dedicated and compassionate individual to join our Community Intervention Team (COMIT) to support individuals with mental health or dementia needs and their caregivers.
Job Description:
Responsibilities:
- Conduct comprehensive assessments of clients’ mental health and dementia needs.
- Develop and implement individualized care plans in collaboration with clients, caregivers, and other healthcare professionals.
- Provide counseling, therapy, and psychoeducation to clients and their families.
- Manage cases by coordinating with various healthcare providers and community resources.
- Monitor and evaluate clients’ progress and adjust care plans as necessary.
- Facilitate support groups and educational workshops for clients and caregivers.
- Maintain accurate and up-to-date documentation of all client interactions and interventions.
- Adhere to COMIT guidelines and protocols to ensure the highest standard of care.
- Participate in team meetings, training sessions, and continuous professional development activities.
Requirements:
- Bachelor’s degree in Social Work, Psychology, Nursing, or a related field.
- Relevant experience in mental health, dementia care, or community healthcare settings.
