PERTAPIS HALFWAY HOUSE provides for treatment and rehabilitative services to substance abusers, enabling them to reintegrate into society and be active contributors.
Halfway House hosts 100 offenders or abusers placed under community based rehabilitation.
Our skills development programs aim to provide the tools for a seamless re-integration in society.
Job Description
• Responsible for the direction, coordination, management, review, implementation and evaluation of resident case management for Pertapis Halfway House (PHH) in alignment with· Responsible for the direction, coordination, management, review, implementation and evaluation of resident case management for Pertapis Halfway House (PHH) in alignment with the organisation’s strategies, policies, procedures and directions of higher management
• Oversees a team of case workers responsible for resident care coordination. Develops and implements case management programs, intake or discharge planning and manages the unit as per the regulatory practices and policies.
• Manage and supervise a team of case workers, identify opportunities for team training and skills advancement and carry out proper training programs for staff in respect of laws, regulations and policies
• Oversee the planning, organising, execution and oversight of the programming aspect for residents.
• Plan and propose relevant rehabilitation and reintegration programmes to ensure that residents are kept meaningfully engaged during their stay at the halfway house.
• Analyse trends and identify emerging needs of residents and develop or arrange for appropriate intervention programmes
• Monitor and maintain programme integrity so as to ensure that programmes are delivered to achieve the best-intended outcome
• Manage the residents safe custody, discipline, needs and track the residents progress during their stay at PHH
• Maintain proper records of admission and discharge register, records of residents personal effects and disciplinary meeting records.
• Ensure all case management records and other relevant documentations of residents are properly maintained and kept updated
• Draft and review the Standard Operations Procedure and Best Practices of the Home in accordance with the Halfway House Service Model (HSM) and policies of the organisation
• Manage and oversee the Individual Care Plan for all residents and prepare the monthly statistical report
• Work closely with Education & Programmes Department and relevant stakeholders to analyse the needs of the residents and to ensure the effective execution of the programme in accordance to the resident’s needs.
• Carry out any other tasks assigned by the Head of the Home, Deputy Head and higher management
Job Requirements
• Degree in Social Work or Psychology OR Accredited Social Worker OR Accredited Social Service Practitioner
• 7 years of experience in the field
• Certified Substance Abuse Counsellor or Certified
Substance Abuse Therapist is Value-Added
• Has supervisory management skills and knowledge
• Excellent interpersonal, verbal, and written communication skills
• Ability to work with all age groups
