- To receive referrals from referral source(s) and prepare them for screening by the Rehab Lead/ Centre Lead/ Nurse
- To schedule appointments for initial assessment, financial counselling and regular therapy sessions
- To perform counselling explaining to clients and/ or their next of kin regarding the contract of service
- To conduct means testing for patients if required
- To prepare and maintain order of case notes for new admissions and for scheduled therapy sessions
- To process admission and discharge of clients
- To enter client’s attendance into the EMR on a daily basis and update the system as necessary
- To liase with Operations & Facilities Management (OFM) personnel for transport arrangement for clients if required
- To liase with the Business Office or Finance personnel for billing matters
- To update the Agency for Integrated Care (AIC) website; EMR and Transport Call Centre (TCC) system as and when necessary
- To perform reception duties, including responding to phone calls and attending to general enquiries
- Carry out other tasks/ projects assigned by the Centre Lead
- To communicate to clients/ next of kin and staff in a professional manner. Communication should reflect consideration of socio-cultural values of the patient, family and significant others
- To maintain absolute confidentiality in matters pertaining to the organization/ departmental/ clients. Confidentiality of the clients’ records is to be maintained at all times in order to maintain privacy of the clients
- Takes responsibility for own learning
- Participates in staff meetings and hospital initiatives
- To liase with external services, statutory bodies and other VWOs as appropriate
Requirements
- Certificate in Healthcare Support (Administration) or equivalent is desirable
- Working in a geriatric set up, long term care or nursing home set up will be an advantage but not necessary
- Service-oriented with client fronting experience
- Meticulous in administrative duties
