Job Scope:
- Reviews patients and assess their suitability for enrolment under Cardiac Rehabilitation Programme (CRP).
- Provides patient and their family information on disease process, causes of disease conditions, health education on risk factors, nutritional counselling, smoking cessation advice, exercise & coping strategies at home following discharge.
- Become point of contact for patient, family and multidisciplinary team.
- Coordinate and schedule Pre-Rehab Assessment clinic and Post Rehab Clinic for patients.
- Coordinate patients’ access and treatment sequence where required for the specialty.
- Input the information into the Rehab Clinic database.
- Conducts nurse-led specialty education session for patients and their families.
- Assist in purchase and work requisition processes.
- Ad hoc duties as assigned by the Head of Department.
Job Requirements:
- Minimum Degree, preferably in Nursing / Allied Health / Science or equivalent
- Excellent interpersonal and communication skills
- Preferably with working experience in a similar setting
- Proficient in using MS office
