As a key member of our Community Partnership team, you will play a vital role in empowering our Seniors of Interest to thrive in their communities. Your primary focus will be identifying, planning, and implementing comprehensive programs that support ageing-in-place, enabling seniors to live independently and confidently. Additionally, you will contribute to successfully executing our Community of Care (COC 2.0) - Go Broader project. This exciting opportunity is based at either AAC-Bukit Merah View or Jurong Central Plaza, where you will join a dedicated team making a meaningful impact in the lives of seniors.
Job Description
Planning and Operations:
- Develop and execute outreach plans and targeted active ageing programmes for Seniors of Interest.
- Support Active Ageing Centres (AACs) in implementing evidence-based initiatives and outreach efforts.
- Assist in evaluating programmes, workflows, and the overall care ecosystem.
- Work with Corporate Communications to develop publicity materials and outreach plans.
Partnership Engagement:
- Implement new initiatives and coordinate partner engagement efforts.
- Create an asset map to identify service gaps and strengths within community partners.
- Collaborate with partners and government agencies, and attend networking events as needed.
Administrative Work:
- Collect and analyse data, document findings, and ensure timely submission of reports.
- Maintain accurate records and support evaluation and reporting for management and funders.
- Perform any additional duties assigned, which may include work on weekends or after hours.
Requirements
- Diploma in Social Work, Community Development, Counselling or Psychology preferred
- At least 2 years experience in programme planning, care coordination, or community work
- Experience in project management, business development or relationship/stakeholder engagement will be an added advantage
Salary and Renumeration
- $3500 - $3800
- 1 month AWS + 2 months Performance Bonus
