Job Title: Social Worker, Sheltered Workshop Programme
The Social Worker will provide support and interventions to persons with disabilities for the Sheltered Workshop Programme. He/She is committed and service-oriented and works in collaboration with the team to support persons with disabilities in providing training and employment support. He/she will work closely with other Allied Health Professionals in supporting persons with disabilities in sheltered employment.
Job Responsibilities
- Conduct and work with other Allied Health Professional for intake assessment, referral, evaluation and discharge of clients in the Sheltered Workshop Programme.
- Maintain case notes, documents and prepare social reports.
- Conduct client’s needs assessment and provide necessary interventions and share with client resources and support systems.
- Work with trainers to provide case management and counselling support to clients.
- Conduct mean test for clients on Enabling Transport Subsidy.
- Responsible for submission of monthly Enabling Transport Subsidy.
- Conduct learning journeys for partners, caregivers to share about sheltered workshop programme.
- To conduct home visits when necessary.
- Work with department Head to plan, implement and evaluate programme to address service gaps.
Requirements
- A Bachelor in Social Work from a recognized university
- Accredited Social Worker, or eligible for professional registration with the SASW
- Able to perform social work administrative functions
- Experience working in a similar setting or with special needs client will be an advantage
- Be able to work independently and with a multi-disciplinary team
- Have passion in wanting to make a difference for people with special needs
Salary Range
· $4000 to $4800
· Working Location: Chai Chee Lane
