Responsibilities
Casework and Counselling
- Maintain at least 80% of active cases per quarter of which 100% to be supported with electronic recordings
- Conduct assessment, planning and intervention for cases
- Participate in internal and external case conferences, networking and case management meetings
- Document and evaluate case outcomes
- Develop and review appropriate intervention strategies
- Adhere to CSWP and SSNet processes in managing cases
Information and Referral
- Provide accurate information and refer clients to appropriate agencies for further intervention
- Attends to walk-in clients
- Adhere to CSWP and SSNet processes in managing referral
Programme – Groupwork / Community Work
- To provide support to Coordinator in planning, implementation, evaluation and documentation of the programme
- To plan and organise events and activities for clients
Requirements
- Degree or Post-Graduate Diploma in Social Work
- Min 3-5 years relevant work experience in Social Work setting
- Demonstrate ability to handle work professionally, independently and efficiently with a strong customer service mindset
- Possess effective organizational and follow-up skills, paying attention to detail
- Good interpersonal skills and able to interface with all levels, internally and externally, in a professional and responsive manner.
