The Role

Responsibilities

• Conduct comprehensive assessments of patients' physical, emotional, and spiritual needs.
• Develop and implement individualized care plans in collaboration with the interdisciplinary team (doctors, social workers, chaplains).
• Administer medication to manage pain and other symptoms.
• Provide emotional support and counseling to patients and their families.
• Educate patients and families about the end-of-life process and available resources.
• Collaborate effectively with other healthcare professionals to ensure seamless care coordination.
• Maintain accurate and confidential patient records.
• Participate in interdisciplinary team meetings and case discussions.

Qualifications

• Registered Nurse (RN) with the Singapore Nursing Board (SNB).
• Valid Post-Registration Nursing Qualification in Palliative Care or a minimum of 2 years of experience in a palliative care setting.
• Strong clinical skills and a deep understanding of palliative care principles.
• Excellent communication and interpersonal skills, with a compassionate and empathetic approach.
• Ability to build rapport with patients and families from diverse backgrounds.
• Emotional resilience and ability to cope with challenging situations.
• Strong teamwork and collaboration skills.
• Proficiency in electronic health records (EHR) systems.

• Conducting comprehensive patient assessments at their homes.
• Developing and implementing individualized care plans in collaboration with patients, families, and physicians.
• Administering medications, including injections, as prescribed by the doctor.
• Performing aseptic procedures such as wound care, catheterization, and ostomy management.
• Monitoring vital signs and reporting any changes to the physician.
• Educating patients and their families on self-care and disease management.
• Providing emotional support and companionship to patients.
• Documenting all care provided in patients' medical records.
• Maintaining a clean and safe environment for patients.
• Collaborating with other healthcare professionals involved in the patient's care plan, such as social workers and therapists.

You are a great fit for this role if you have:

• Registration as a Nurse with the Singapore Nursing Board.
