An exciting career opportunity has arisen at AMK-THK Hospital for full-time Senior Occupational Therapist to join our integrated care team. We are looking for outgoing and flexible individuals with proven experience of working in the area of adult rehabilitation. You will prepare our patients for community integration through carefully designed activities and processes. You are required to demonstrate sound assessment, clinical reasoning and complex discharge planning skills. Your work also involves providing home assessment and advice on home modifications.

The hospital provides full suite of rehabilitation to patients with Stroke, Fracture, Post-surgical deconditioning etc. The rehab unit has a well-established multi-disciplinary team approach and the ideal candidate needs to demonstrate excellent communication and interpersonal skills.

This post offers an excellent opportunity for someone who wishes to develop their clinical expertise in the areas of Stroke, Dementia, frail elderly or fractures. They will have the support of the multidisciplinary team. In addition, the candidate will work alongside other therapists, therapy assistants and students.

Requirements:

• Minimum a recognised degree in Occupational Therapy
• Obtained Full Registration with AHPC
• Possess relevant years of clinical practice experience

Responsibilities:

• Perform medication reconciliation & review, and ensure the appropriateness of patient drug therapy.
• Dispense medications and give appropriate counselling to patients and caregivers.
• Provide timely and accurate drug information to healthcare professionals and patients / caregivers.
• Be involved in educating and training of pharmacy staff and other healthcare professionals.
• Participate in projects/activities which support the goals of the department and hospital.

Requirements:

• Degree in Pharmacy.

The Nurse Manager manages and guides community nurses and care staff in order for them to provide holistic clinical care to clients and their caregivers at home to a certain standard, to allow clients to keep well in the community and minimize need for admission to the hospital.

Responsibilities

1. Clinical Quality and Assurance

• Ensure staff care plans and follow-ups are carried out in accordance to expected standards to deliver good clinical outcomes.
• Plan developmental needs and competencies of clinical staff based on operational requirements.
• Review and maintain clinical SOPs and policies with Programme Manager/Director to ensure compliance with MOH service requirements for non-residential ILTC services.
• Review and follow up on escalated clinical incidents with Programme Manager/Director.

2. Team Management & Preceptorship

• Provide direction and guidance on staff performance and conduct to ensure clarity of staff expectations.
• Provide direction and guidance on care planning of complex cases and/or cases in alignment to the aims of the Home Healthcare team as set out by Programme Manager/Director.
• Delegate caseload appropriately in accordance to staff competency.
• Ensure staff licenses, trainings and competencies are kept up to date.

3. Project Management

• Assist the Director/Programme Manager in planning, implementing and evaluating the Programme’s operations and programmes to meet the clinical needs of home personal care, interim caregiving, home medical and home nursing services.
• Provide guidance and supervision to clinical staff in carrying out quality improvement projects.

Job Purpose:

a) To provide counselling and psychosocial support for patients and families through case management, in-depth assessment of family dynamics, linking up with required social service agencies to facilitate care and discharge planning, for inpatients and outpatients.

b) To contribute to the community via supporting colleagues in needs assessment, program development, implementation and/or evaluation of programs.

Responsibilities:

1.Clinical and Case Management:

i. Engage and build professional relationship with clients and significant others.

ii. Utilize specialized theories in engaging with clients and significant others (MSW I).

iii. Gather relevant information through intake assessment to determine needs and evaluate psychosocial status of clients and families.

iv. Develop, implement and follow up appropriate care plans with clients and families.

v. Manage and overcome reluctance and resistance to change (MSW I).

vi. Conduct financial assessment to determine needs for financial assistance, when required.

vii. Provide illness- related psychoeducation, counsels and support clients/families experiencing psychosocial issues associated with the illness.

viii. Conduct home visits when appropriate and necessary.

ix. Establishes and maintains a working relationship with the multidisciplinary team through regular conferences.

x. Network, refer and liaise with service providers, agencies to match clients to appropriate community services to ensure continuity of care.

xi. Assist in data collection and support action-based research in accordance with clinical practices.

xii. Adhere to protocols in practice guidelines and integrated care pathways.

2. Professional Development:

i. Engage in continual learning through regular case discussions, supervision and trainings (internal and external).

ii. Provide orientation to new staff.

iii. Guide new and fresh staff (MSW I).

iv. Provide sharing sessions on social work for students and professionals (MSW I).

v. Supervision of social work interns (MSW I).

3. Programme Development:

i. Assist and participate in the needs assessment, planning, delivery, monitoring and evaluating of programs when appropriate.

ii. Support psycho-educational groups for patients and/or caregivers.

iii. Develop and lead in support groups for patients within and outside the organization (MSW I).

iv. Assist in facilitating therapeutic group-work when appropriate (MSW I).

v. Deliver public education talks and conduct training above foundational level (MSW I).

vi. Identify service gaps in response to clients’ needs and make recommendations to improve service provision of programmes (MSW I).

4. Administrative:

i. Ensures proper and prompt documentation of case notes, referrals and other related documents for case management and audit purposes.

ii. Collect and submit regular statistics.

iii. Carry out relevant administrative duties as assigned by Supervisor.

Requirements:

• Education/ Training/ Certification: Bachelor in Social Work / Post Graduate Diploma in Social Work.
• Experience: Fresh graduates are welcome (MSW II).
• Minimally 5 years of clinical experience for MSW I.

Assist clients and their caregivers to achieve a seamless transition from the various stages of the care journey (e.g. Hospital to Home) by providing them with customer centric and holistic support services.

Responsibilities

• Conducts screening referrals and assessment of potential clients and the home environment.
• Develop and perform an “Individualised Care plan” (ICP) involving the caregivers / family members (if possible) for clients.
• Monitor clients’ conditions and conduct periodical review of clients’ ICP and conditions.
• Conduct financial counselling and provide advice on the application of government subsidies.
• Collects payments and issues receipts for service delivered.
• Manage hotline (on rotation).
• Assists Doctors during the home visits.
• Develop “Discharge Plan” for clients who do not require the service.

Requirements

• Candidate must possess at least a Diploma/Bachelor's Degree in Nursing or equivalent.
• Registered with the Singapore Nursing Board
• At least 3 year(s) of working experience in the related field is required for this position.

Enrolled Nurse (Renal)

Responsibilities

• Participate in the assessment, implementation and evaluation of quality dialysis outcome.
• Perform Hemodialysis cannulation and treatment for patients according to the standards practice and guideline.
• Carry out Hemodialysis treatment and venipuncture on arterio-venous fistula, arterio-venous graft and temporary catheter prescribed for patient according to the expected standards of practice and guidelines.
• Demonstrate and maintain good interpersonal skills with colleagues, patients and public.
• Provide diet education and care of the vascular access to patient and family.
• Work in collaboration with Staff Nurse to prioritize and organize care to meet the dialysis adequacy and rehabilitative needs of the patients.
• Assist the doctor and nursing staff during Hemodialysis treatment and examination review of the patient.
• Provides orientation, guidance, mentoring and coaching to new and less experienced staff.
• Collect water sampling for water culture to maintain safe water standard guideline.
• Review new inpatient and outpatient latest virology blood result from admission office.

Requirements

• Candidate must possess at least a NITEC in Nursing.
• At least 1 year of relevant working experience in a dialysis centre.
• Valid registration with Singapore Nursing Board.

Staff Nurse (Renal Dialysis)

Responsibilities

• Participate in the assessment, implementation and evaluation of quality dialysis outcome.
• Perform Hemodialysis cannulation and treatment for patients according to the standards practice and guideline.
• Practise proper infection control procedures based on current infection control guidelines.
• Guide and direct the enrolled nurse level and below, to ensure that quality nursing care is rendered to patient at all times.
• Record informative, accurate and concise statement on the Hemodialysis treatment, blood results and Parameters.
• Administer intravenous antibiotics, IV Erythropoietin and Blood Transfusion during Hemodialysis treatment with standard of practice.
• Collect water sampling for water culture to maintain safe water standard guideline.
• Work closely together with respective hospital for blood investigation and follow up.
• Work closely with Dietician and educate patient on nutritional status and fluid compliance.
• Take appropriate nursing actions in emergency situations that compromise patient safety.

Requirements

• Diploma in Nursing recognized by Singapore Nursing Board.
• At least 2 years of relevant working experience in dialysis center.
• Valid registration with Singapore Nursing Board.

Responsibilities
