Job Description:

• The Case Management Officer works with clinical departments to develop and implement clinical pathways and case management programmes and activities. These include assessing, planning, coordinating, monitoring and evaluating of clinical pathways and case management services.

Responsibilities and Duties

You will be responsible for the following:

• Screen all new admissions daily to identify cases for case management / receive referral from the ward.
