Responsibilities:

1. Provide home-based psychosocial rehabilitation, follow-up on treatment and support to patients and their caregivers in the community.
2. Plan, implement and document effective and comprehensive care plan with the multidisciplinary team for client on identified problems/needs.
3. Monitor clients’ progress, supervise their medication and administer injections as prescribed by the doctor.

Requirements:

1. Registered Nurse with SNB
2. Possess a Registered Mental Health Nurse /Adv Dip Mental Health or other relevant qualifications (e.g. social work, counseling, psychology)
3. Working experience in psychiatric setting (with Adv Dip in Mental Health) with at least 6 months of experience in acute setting, or experience in psychiatric setting for at least 4 years and 6 months of experience in acute setting
4. Passion to work in the community
5. Possess good interpersonal, communication, writing & IT skills
6. Familiar with healthcare environment
7. Able to work independently with good decision-making skills
