Job Description:

• You will work at Department of Urology, Singapore General Hospital, to help manage clinical web-based databases
• You will abstract clinical data on patients who were diagnosed and/or treated at Department of Urology, Singapore General Hospital
• You will export and clean clinical data from eHIntS
• You will enter / import clinical data into the web-based databases and keep the databases updated and maintained
• You are to ensure timeliness, completeness and accuracy in case-finding, data abstraction and data entry
• You will fulfill necessary assistance in submissions of IRB applications / renewals at the Department of Urology SGH
• You will contribute to research outcomes of the projects involved and of the Department.
• You will assist in administrative duties in the Department and perform duties outside of regular job scope from time to time as required by the Principal Investigators of clinical databases and Head of Department of Urology.

Job Requirements:

• Bachelor degree in Nursing, Healthcare, Bio-medicine related;
• Competent in Microsoft Excel;
• Statistic background/Programming knowledge in Python/R is an advantage.

The Program Coordinator will be involved in the care of respiratory patients in Singapore General Hospital with chronic respiratory conditions particularly on early diagnosis of COPD and smoking cessation programme and the main responsibilities consist of screening, planning, assessment and monitoring of patients’ admissions, conducting patient education and counselling, and training of other health care professionals. The incumbent will be required to participate in training and education programmes, and research activities.

Responsibilities

• Co-ordinates inpatient/outpatient service together with nurse clinicians and Allied Health Professionals
• Screening inpatient & outpatient smoker referral for smoking cessation for symptoms and offer portable spirometry for early COPD diagnosis
• Train and perform portable spirometry
• Provide COPD, airway disease and smoking cessation counselling both inpatient and outpatient
• Provides advice on the pharmacotherapy for smoking cessation
• Provides follow up for patients
• Participates and coordinate research related to early COPD/COPD/smoking cessation

Requirement

• Bachelor in Nursing with 2 years’ relevant work experience
• Experience in counselling and having a Certification in Quit Smoking Counselling would be an advantage
• Ability to work in a multi-disciplinary team consists of other healthcare professionals
• Having a Good Clinical Practice Certificate for clinical trials would be an advantage
• Knowledge of MS Office is compulsory

The Hepatopancreatobiliary (HPB) Coordinator manages and supervises the operational and administrative functions and activities of the HPB department. He/She is responsible for overall coordination and care of HPB patients’ pre-and post-operative management. He/She will provide patient counselling, develop education materials, and conduct talks for medical and nursing staff. In addition, he/she will also coordinate various research activities involving HPB; as well as oversee the maintenance of databases of patients.

Responsibilities

• Provides knowledge, counselling and teaching to patients and family on HPB diseases, pre and post-operative procedures and treatment options

• Supervision of nursing care of the HPB patients (pre-operation, ICU and general ward and in follow-up clinic).

• Overall responsible for the HPB Enhanced Recovery After Recovery Programme: Planning / Protocol evaluation / Performing pre-operative education to the patients and families / Monitoring and Evaluation of the programme / Data entry

• Responsible in the coordination with the multidisciplinary team, especially in the care and management of HPB patients undergoing Interventional Procedures

• Plan, implement and evaluate patient and family educational guidelines, teaching guides and pertinent educational materials needed for patient and family education in the department.

• Responsible in the coordination of patient care management in the outpatient settings

• Assist in the coordination of patient care management in the HPB Pancreas Clinic.

• Assist the Head of Department in planning, recommending and monitoring of the goals of the department

• To engage CGH’s internal clinical and operational business units from SingHealth institutions and MOH/AIC to identify care integration needs, conceptualise, develop and implement new initiatives.
• To manage and guide fact-finding, population health database and predictive tool development to enable business units to develop appropriate actionable measures to right site patients to appropriate care setting.
• To monitor established programmes and to work closely with programme managers to ramp up programmes and ensure their continued relevance.
• To identify, engage and build strong partnerships with community healthcare services and partners including Primary Care, SACs, allied health and home nursing services and other relevant social services and community-based organizations.
• To facilitate building of capabilities of primary and community care partners through identification and implementation of key programmes and initiatives. To identify workable operational models to achieve eventual benefit of the patient.
• To identify, engage and build strong partnerships with appropriate and relevant industry partners in seeking novel and financially-viable options of implementing new care integration initiatives.

Job Description

The Nutrition Support Service clinical coordinator provides the following care services:

• Care coordination, education and management of home parenteral nutrition (HPN) and enteral tube feeding.
• Clinical care and support to encourage full rehabilitation of patients on both HPN and enteral feeding under NSS and also responsible for coordinating their follow up care.
• Counselling to patients and their families on both home parenteral and enteral feeding process.
• Ordering and basic interpretation of diagnostic and laboratory tests
