The podiatrist assistant is an integral part of the podiatry unit and perform various basic podiatry care and podiatry treatment I procedures in order to meet the needs of patients under supervision. These include diabetic foot screening, basic foot

care, community outreach and assisting the podiatrists in their roles and responsibilities.

As a Podiatrist Assistant, job scope will perform clinical care and direct patient management under the supervision of the podiatrists.

Major Duties and Responsibilities

A. Clinical Competencies & duties

• In podiatry services, the Podiatrist Assistant will function alongside podiatrists t provide services to the patients. The Podiatrist Assistant can perform certain procedures under supervision.
• Assists podiatrists in day-to-day operations, in patients, outpatients and in the community
• Assists podiatrists in clinic operations, manufacture, and modification of products for dispensation to patients
• Assist podiatrists in nail surgery procedures in outpatients
• Assist in transfer of patients to treatment couch and escorting patients in and out of treatment room.

B. Administrative duties:

• Assist podiatrists in patient appointment scheduling and re-scheduling.
• Maintain and ensure general tidiness and cleanliness of all treatment rooms and plaster rooms, as well as all equipment and devices.
• Manage enquiries from public and internal staff.
• Engage in relevant continuing education and training on regular basis to maintain his / her competency.
• Participate in corporate activities and events.
• Adhere to all policies of the department in upholding quality service in patient care.
• Participate in departmental process and quality improvement efforts (E.g. kaizen projects, 5S etc)
• Support podiatrists in specific department al portfolios (E.g.: equipment and instrument management, continuing education, 5S champion, Kaizen champions, JCl compliance etc).
• Work as an effective team player within the department and also outside the department.
• Embrace continuous improvements as our way of life by improving and refining work performance on a daily basis.
• Assist HOD in administrative duties when assigned.
• Undertake all duties when assigned.

JOB REQUIREMENTS

EDUCATION TRAINING AND EXPERIENCE

• Minimum GCE 'N', '0' Levels, ITE or equivalent and I or
• Skills in word processing software (E. g. : Microsoft word)
• Related on the job-training by the hospital

The Executive / Senior Executive will be responsible to maintain a high level of service standards in the department of Housekeeping. To assist the Executive Housekeeper in the daily running of the department and maintain a high level of service to the patients and to ensure continuous improvements to work processes for both Khoo Teck Puat and Yishun Community Hospital.

MAJOR DUTIES AND RESPONSIBILITIES

(A) SPECIFIC

1. Assist to ensure department operates and complies with applicable laws and regulations.

2. Assess, identify and mitigate risks that may hinder operations, reputation, safety and financial well-being of the organisation.

3. Assist to ensure review of department policies, inventory, maintenance of equipment, monitor feedback and manage operational issues, for high levels of work quality.

4. Assist to ensure optimal use of resources, develop and implement plans to attract, train and retain essential skills, and to motivate staff to bring out the best in them and contribute to organisational goals.

5. Assist to ensure staff are well-trained and adhere to regulatory, safety and infection control standards.

6. Assist to ensure smooth communication between hospital management, feedback from customers, mandatory authorities, vendors etc.

7. Identify and resolve housekeeping problems encountered by the Environmental Service Associates (ESAs).

8. Walk the ground to ensure that a consistent high level of service is maintained including YH Standards.

9. Manage ground feedback and complaints with an understanding of the mechanisms of the feedback channels so that work processes can be improved.

10. Foster continuous improvement as our way of life by encouraging staff efforts at improving processes to achieve operational excellence.

JOB REQUIREMENTS

(A) EDUCATION, TRAINING AND EXPERIENCE

• Diploma and above or minimum 3 – 5 years in service industry/ housekeeping/

healthcare related areas.

• Experienced in a supervisory role is necessary and important.
• Knowledge of hospital and service operations would be preferred.
• Good communications and interpersonal skills.

The Aftercare Case Manager (Assistant Manager) will support the KTPH PsyMed team by providing comprehensive support and guidance to clients with psychiatric disorders during their transition from hospital setting to community-based services.

DEPARTMENT FUNCTION(S)

The main duties and responsibilities of the ACM include:

• Practicing case management in line with the established inclusion/exclusion criteria.
• Co-developing clinical pathways, workflows, escalation protocols, and patient stratification with input from multidisciplinary teams.
• Applying clinical expertise and resource knowledge to identify and refer patients to appropriate support services.
• Ensuring effective information flow among stakeholders, serving as a point of contact between KTPH and community partners.
• Facilitating regular case discussions with stakeholders to co-create solutions with the aim of achieving optimal patient outcomes.

Case Management

• Screen and identify suitable patients for enrollment into the Aftercare program based on established criteria.
• Collaborate closely with patients to assess needs and preferences for aftercare, ensuring active involvement in care planning.
• Provide psychoeducation and support to patients and families regarding treatment options and community resources.
• Serve as a point of contact for consulting physicians regarding patient concerns or updates.
• Review care plans (including medication management and therapy referrals) to ensure transitional support. Monitor follow-up appointments to ensure continuity of care and address barriers to attendance, if any.
• Cultivate strong partnerships among stakeholders and harness their collective resources and expertise to address patients’ complex needs and facilitate comprehensive support services for transitioning patients.
• Guide patients’ seamless reintegration into the community by coordinating tailored care plans and facilitating access to stability and recovery services.
• Foster proactive collaboration with multidisciplinary teams and stakeholders to ensure effective patient care across healthcare settings, promoting long-term recovery and well-being.
• Conduct home visits and interact with patients’ family members/caregivers when needed.
• Empower patients to manage their mental health effectively and make informed decisions about their care.
• Participate in discussions and case conferences with community partners to share knowledge and best practices.

Data Management and Administrative Duties

• Maintain accurate and confidential records of assessments and screenings for patients in electronic health record systems (EPIC), ensuring completeness and compliance with privacy regulations.
• Utilize data management tools to analyze trends and patterns in patient outcomes and program effectiveness.
• Generate reports to communicate findings and guide quality improvement initiatives.
• Conduct regular audits to ensure data integrity and compliance with regulatory standards.
• Oversee intake and assignment of referrals from hospital clinicians to the Aftercare program.
• Coordinate with community partners to streamline the referral process and ensure smooth transitions for patients.
• Schedule and organize meetings, conferences, and case discussions.
• Ensure timely communication and collaboration among participants.

JOB REQUIREMENTS

(A) EDUCATION, TRAINING AND EXPERIENCE

a. Knowledge – Mental Health, Behavior science, suicide assessment and intervention, case management principles, familiarity with professional and technical emerging knowledge, community resources and governmental policies that impact the patients and basic counselling techniques.

b. Skills – good interpersonal communication, advocacy, problem solving and administrative duties.

i. Communication skills: to be able to communicate effectively and listen actively. Case managers must also be aware of nonverbal communication cues and be able to read nonverbal cues with patients.

ii. Interviewing skills: to be able to evaluate the patients’ needs. Case managers need to be able to formulate questions, record client information and answer patients or family members’ questions promptly.

iii. Teaching skills: to be able to help patients develop skills and knowledge to improve their life situation. Case managers need to help patients develop social skills, regain independence, identify and build coping strategies etc.

iv. Advocacy Skills: case managers have to engage in advocacy on behalf of their patient. Case managers may attempt to resolve conflicts between patients and services providers and work towards protecting the best interests of the patients.

v. Organisational skills: Case managers have to be adept at planning and organisation to ensure service delivery to their patients in a timely manner.

vi. Aptitude for teamwork and ability to work well with diverse groups, from other professionals to patients’ family members. Case managers don’t work alone to help their patients improve their quality of life. Instead they help patient achieve their goals as part of a much larger team.

d. Personal characteristics

i. Desire to help others: case managers must have a strong desire to help others or the negative aspects of the job can quickly lead to burnout.

ii. Patience – case managers have to deal with a lot of red tape in the course of programs/ service implementation and in supporting patients.

iii. Empathy: to be able to identify with the struggles patients are having.

i. Degree or equivalent experience – Degree in Psychology, Counselling, or Nursing

ii. Years of Experience: 3-4 years in a mental health, either in community or healthcare environment.
