Our Hospital Client is seeking a dedicated and compassionate Medical Social Worker to join their team. This role focuses on supporting patients with skin conditions through comprehensive case management, collaboration with a multidisciplinary team, and community outreach.

Job Responsibilities

• Casework management and support with biopsychosocial-spiritual assessment & intervention
• Collaborate with a multi-disciplinary team to refer patients to suitable programmes for financial assistance.
• Participate or assist in developing, conducting and evaluating patients’ support groups and community outreach programmes.
• Deliver psychoeducation to patients, families, staff and members of the public through workshops & seminars.
• Participate in organization/department initiatives and projects

Job Requirements:

• Degree in Social Work/ Post Graduate Diploma in Social Work with 2 years of social work experience
• Candidates with a Post-Graduate Diploma in Counselling will have an added advantage
• Effective clinical skills in assessment, intervention, casework and counselling
