A community hospital is seeking a compassionate and dedicated Home Care Staff Nurse to join our growing team. In this role, you will play a vital part in providing high-quality nursing care to patients in the comfort of their own homes. You will work autonomously while collaborating with a team of healthcare professionals to ensure patients receive the care they need to thrive.
About the Role
This position is ideal for a registered nurse who thrives in a fast-paced environment and enjoys building relationships with patients. As a Home Care Staff Nurse, you will work office hours, from 9am to 6pm, Monday to Friday. Your responsibilities will include:
- Conducting comprehensive patient assessments at their homes.
- Developing and implementing individualized care plans in collaboration with patients, families, and physicians.
- Administering medications, including injections, as prescribed by the doctor.
- Performing aseptic procedures such as wound care, catheterization, and ostomy management.
- Monitoring vital signs and reporting any changes to the physician.
- Educating patients and their families on self-care and disease management.
- Providing emotional support and companionship to patients.
- Documenting all care provided in patients' medical records.
- Maintaining a clean and safe environment for patients.
- Collaborating with other healthcare professionals involved in the patient's care plan, such as social workers and therapists.
You are a great fit for this role if you have:
- Registration as a Nurse with the Singapore Nursing Board.
- Minimum of 3 year experience in a clinical setting, preferably in home care or geriatrics.
- Strong assessment and clinical skills.
- Excellent communication, interpersonal, and organizational skills.
- Compassionate and patient-centered approach to care.
- Ability to work independently and as part of a team.
- Proficiency in computer documentation.
